Publication: Making Government-Led Risk and Crisis Communication More Effective: Lessons Learned From Six Countries During the COVID-19 Pandemic

Abstract
Government-led risk and crisis communication (RCC) is an essential priority in directing and supporting effective pandemic response in a country. Ineffective RCC during public health emergencies such as the COVID-19 pandemic can lead to public confusion, fear, and distrust, which eventually become barriers to the acceptance and practice of risk preventive behavior. In this analysis of five key RCC objectives in six countries (Ghana, Republic of Korea [Korea], Singapore, the United States [US], the United Kingdom [UK], and Vietnam), several lessons for future consideration are identified. First, governments ought to explore reasons for not being able to rapidly initiate and implement effective RCC so barriers can be identified and overcome before another crisis emerges. Second, public leaders must be committed to enhancing consistency, transparency, and accountability in their messaging, by prioritizing deference toward expert advice from scientific and public health experts. Third, it would be prudent to evaluate laws, standard operating procedures, and existing workflows to enhance public-private partnerships that enable cross-utilization of multistakeholder expertise and resources during a crisis. With this goal, it may be useful to proactively identify and acknowledge the important role that information and communication technologies (ICT) can play during non-pandemic times in enhancing health literacy and health communication. Strengthening laws and regulations to ensure privacy and human rights protections for those using ICT should be an ongoing priority, so that when ICTs must be leveraged during a crisis, there is a baseline level of assurance and confidence surrounding their safe and responsible use. Efforts to encourage the familiarization of the public with how infectious disease outbreaks spread and how people can keep themselves and each other safe should also be an ongoing priority, as scientific and health literacy can help the public prepare itself to receive and interpret RCC that is provided during a crisis. The continuous exposure to the technologies and means of sharing health information throughout various subpopulations within a country is an essential part of trust building over time, thus mitigating the fear, anxiety, social stigma, and potential distrust that could occur in times of an evolving pandemic, when levels of uncertainty are still quite high. Routine, repeated assessments of the public’s perceptions and needs during a crisis are key, as are other efforts to increase a nation’s preparedness to launch RCC rapidly and effectively (for example, government commitment to exploring public private partnerships).

Publication Effective Responses to Non-communicable Diseases : Embracing Action Beyond the Health Sector(World Bank, Washington, DC, 2011-09)Effective responses to non-communicable disease: Embracing action beyond the health sector focuses on solutions, indicating opportunities for the prevention and control of non-communicable diseases (NCDs) and the kinds of actions that will achieve it. NCDs exact a heavy toll on individuals and society. They cause disease, disability, and death, and reduce productivity which is vital for development. They also impose hefty costs on health services, particularly since NCDs frequently lead to ongoing disability and need for long-term care. Prevention that results in healthy aging and the reduction of morbidity is far more cost-effective and financially sustainable than treatment alone. A small number of proven prevention measures could stem the rise of the NCD epidemic. Data indicate the potential of affordable, potent, prevention tools focused on five key areas: tobacco, alcohol, diet, exercise and nutrition. Together, programs targeting these areas could reduce the burden of NCDs by more than half, while costing only a tiny fraction of current health spending. Since successful prevention involves multiple sectors and actors, countries will need to adopt a framework that clearly sets out the different levels of accountability of each role player. Such a framework will require strong national buy-in, but there are tools, examples, and support agencies available to facilitate this. Middle- and low-income countries can seize the opportunity and act now to tackle NCDs and the underlying risk factors of these diseases. If they do so, these countries will greatly increase their prospects of reaping the full benefits of their demographic dividend, and of ensuring sustained economic development, greater equality, and a better quality of life for their people in the years ahead.Publication The Challenge of Non-Communicable Diseases and Road Traffic Injuries in Sub-Saharan Africa : An Overview(World Bank, Washington, DC, 2013-06)This report draws on a comprehensive review of the literature and on input from policy makers, researchers, and practitioners to address four questions: (1) how is the growing burden of non-communicable diseases (NCDs) and road traffic injuries (RTIs) changing the epidemiology of Sub-Saharan Africa? (2) What determines and drives this burden, and what are the commonalities with communicable diseases? (3) What is the rationale for public intervention? (4) How could resource-constrained governments approach NCD prevention and treatment and road safety in a comprehensive, effective and efficient way? The data show that action against NCDs and RTIs in Sub-Saharan Africa is needed, together with continued efforts to address communicable diseases and maternal and child health as well as to reach the Millennium Development Goals (MDGs). The report suggests that NCDs and RTIs should not be tackled separately as a vertical program, nor should they displace communicable diseases as priorities. Instead, given resource constraints, and some shared determinants, characteristics, and interventions, there is scope for an integrated approach focusing on functions (prevention, treatment, and care) rather than on disease categories. Examples are cited of potential opportunities to integrate and add NCD prevention and treatment into existing services and programs. Proven, cost-effective, prevention interventions are clearly needed, many of which (such as tobacco and alcohol taxes, road safety measures, and fuel-efficient ventilated cook-stoves) require action beyond the health sector. These can deliver broader development benefits in addition to their benefits for health. Selective, evidence-based actions to reduce NCDs and RTIs will address the changing disease burden in Africa and achieve a more sustainable improvement in health outcomes, more efficient use of resources, and better equity across patients and populations.Publication Rapid Assessment of the Effect of the Economic Crisis on Health Spending in Mongolia(World Bank, Washington, DC, 2010-12)This rapid assessment examines the effect of Mongolia's economic crisis on government health budgets and health expenditure, household out-of-pocket spending and donor health commitments. This study was part of a larger assessment conducted in four countries on the effects of the economic crisis on health spending. A standardized approach was developed for all country case studies and consisted of a desk review of internationally-available literature and databases, extensive in-country review of data and documents available in government and donor offices, and semi-structured interviews with government staff, health providers and development partners. This assessment in Mongolia reveals a substantial reduction in the government health budget: the 2009 national health budget was significantly lower than the previous year's, and then was further reduced by 10 percent in a subsequent budget amendment. At national level, budget cuts were concentrated in investment line items. Among recurrent line items, the pharmaceutical budget was hardhit, but salaries were largely preserved, and there were no retrenchments. Similar patterns were observed at sub-national level for hospital budgets, which depend on the central allocations, but not for primary care facilities, which are funded on a capitation basis. Compared to other sectors, the health sector was relatively protected during the economic crisis and the share of health in the total government budget was higher after the budget amendment than before. To protect households from the effects of the economic crisis on health spending, the government undertook specific policy measures to expand health insurance coverage to vulnerable groups. Donor commitments to the health sector during the crisis largely tracked previously planned commitments.Publication HIV/AIDS(Washington, DC, 2003-10)The epidemic spread of HIV/AIDS has been ferocious, posing a great threat, not only to public health, but to social sectors, and to development itself, while the fiscal cost of HIV/AIDS is significant as well. A set of effective prevention interventions include: changing behavior through communication; making the use of condoms, diagnosis, and treatment of sexually transmitted diseases, including counseling and testing, available and affordable; ensuring a safe blood supply; and, preventing parent-to-child transmission. In addition, countries should consider implementing programs to provide cost-effective management of common opportunistic infections. Similarly, community-based, and home-based care should be made available, in addition to traditional hospital care, particularly in countries with a generalized epidemic. Lessons convey the vital need for early action, supported by community participation, targeting the most vulnerable, and prioritizing interventions, while governments should be committed to its prevention, control, and funding. Additionally, nongovernmental organizations' participation plays a decisive role against the epidemic spread, in their capacity for social mobilization, and in reaching marginal groups.Publication Fiscal Challenges after the Global Financial Crisis : A Survey of Key Issues(World Bank, Washington, DC, 2014-03)The global financial crisis and the response to it have contributed to a sharp increase in public indebtedness in a large number of countries. While there have been episodes of high debt in the past, there are a number of long-term challenges today that are likely to complicate the implementation of sustainable fiscal policies in the coming years. Population aging and climate change are factors that are likely to contribute to rising fiscal pressures and the crisis has highlighted the risks and vulnerabilities stemming from reduced fiscal space. This paper argues that heightened fiscal challenges can only be dealt with successfully by adopting a long-term fiscal planning horizon. The paper analyzes a range of available policy tools that countries have used in the past to improve fiscal management. Particular attention is paid to the role of rules-based policies, improvements in the budget process, better accounting of long-term liabilities in the government budget, the deleterious effects of unproductive expenditures, and the painful trade-offs created by the crisis and the toolkit at hand to address them.

Publication World Development Report 2006(Washington, DC, 2005)This year’s Word Development Report (WDR), the twenty-eighth, looks at the role of equity in the development process. It defines equity in terms of two basic principles. The first is equal opportunities: that a person’s chances in life should be determined by his or her talents and efforts, rather than by pre-determined circumstances such as race, gender, social or family background. The second principle is the avoidance of extreme deprivation in outcomes, particularly in health, education and consumption levels. This principle thus includes the objective of poverty reduction. This Report offers practical insights for policymakers, executives, scholars, and all those with an interest in economic development.Publication Classroom Assessment to Support Foundational Literacy(Washington, DC: World Bank, 2025-03-21)This document focuses primarily on how classroom assessment activities can measure students’ literacy skills as they progress along a learning trajectory towards reading fluently and with comprehension by the end of primary school grades. The document addresses considerations regarding the design and implementation of early grade reading classroom assessment, provides examples of assessment activities from a variety of countries and contexts, and discusses the importance of incorporating classroom assessment practices into teacher training and professional development opportunities for teachers. The structure of the document is as follows. The first section presents definitions and addresses basic questions on classroom assessment. Section 2 covers the intersection between assessment and early grade reading by discussing how learning assessment can measure early grade reading skills following the reading learning trajectory. Section 3 compares some of the most common early grade literacy assessment tools with respect to the early grade reading skills and developmental phases. Section 4 of the document addresses teacher training considerations in developing, scoring, and using early grade reading assessment. Additional issues in assessing reading skills in the classroom and using assessment results to improve teaching and learning are reviewed in section 5. Throughout the document, country cases are presented to demonstrate how assessment activities can be implemented in the classroom in different contexts.Publication Digital Africa(Washington, DC: World Bank, 2023-03-13)All African countries need better and more jobs for their growing populations. "Digital Africa: Technological Transformation for Jobs" shows that broader use of productivity-enhancing, digital technologies by enterprises and households is imperative to generate such jobs, including for lower-skilled people. At the same time, it can support not only countries’ short-term objective of postpandemic economic recovery but also their vision of economic transformation with more inclusive growth. These outcomes are not automatic, however. Mobile internet availability has increased throughout the continent in recent years, but Africa’s uptake gap is the highest in the world. Areas with at least 3G mobile internet service now cover 84 percent of Africa’s population, but only 22 percent uses such services. And the average African business lags in the use of smartphones and computers as well as more sophisticated digital technologies that catalyze further productivity gains. Two issues explain the usage gap: affordability of these new technologies and willingness to use them. For the 40 percent of Africans below the extreme poverty line, mobile data plans alone would cost one-third of their incomes—in addition to the price of access devices, apps, and electricity. Data plans for small- and medium-size businesses are also more expensive than in other regions. Moreover, shortcomings in the quality of internet services—and in the supply of attractive, skills-appropriate apps that promote entrepreneurship and raise earnings—dampen people’s willingness to use them. For those countries already using these technologies, the development payoffs are significant. New empirical studies for this report add to the rapidly growing evidence that mobile internet availability directly raises enterprise productivity, increases jobs, and reduces poverty throughout Africa. To realize these and other benefits more widely, Africa’s countries must implement complementary and mutually reinforcing policies to strengthen both consumers’ ability to pay and willingness to use digital technologies. These interventions must prioritize productive use to generate large numbers of inclusive jobs in a region poised to benefit from a massive, youthful workforce—one projected to become the world’s largest by the end of this century.Publication Argentina Country Climate and Development Report(World Bank, Washington, DC, 2022-11)The Argentina Country Climate and Development Report (CCDR) explores opportunities and identifies trade-offs for aligning Argentina’s growth and poverty reduction policies with its commitments on, and its ability to withstand, climate change. Given Argentina’s context, this CCDR focuses on win-win policies and investments, which have large co-benefits or can contribute to raising the country’s growth while helping to adapt the economy, also considering how human capital actions can accompany a just transition.Publication Lebanon Economic Monitor, Fall 2022(Washington, DC, 2022-11)The economy continues to contract, albeit at a somewhat slower pace. Public finances improved in 2021, but only because spending collapsed faster than revenue generation. Testament to the continued atrophy of Lebanon’s economy, the Lebanese Pound continues to depreciate sharply. The sharp deterioration in the currency continues to drive surging inflation, in triple digits since July 2020, impacting the poor and vulnerable the most. An unprecedented institutional vacuum will likely further delay any agreement on crisis resolution and much needed reforms; this includes prior actions as part of the April 2022 International Monetary Fund (IMF) staff-level agreement (SLA). Divergent views among key stakeholders on how to distribute the financial losses remains the main bottleneck for reaching an agreement on a comprehensive reform agenda. Lebanon needs to urgently adopt a domestic, equitable, and comprehensive solution that is predicated on: (i) addressing upfront the balance sheet impairments, (ii) restoring liquidity, and (iii) adhering to sound global practices of bail-in solutions based on a hierarchy of creditors (starting with banks’ shareholders) that protects small depositors.
