Abstract
Costa Rica was one of the first countries in the Latin America and Caribbean region to choose the World Bank Program for Results (PforR) financing instrument to support the implementation of the Strategic Agenda for Strengthening Health Insurance by the Costa Rican Social Security Fund (CCSS), for its name in Spanish, Caja Costarricense de Seguro Social). The PforR’s unique features include using a country’s own institutions and processes and linking disbursement of funds directly to the achievement of specific program results, which helps building capacity within the country, enhances effectiveness and efficiency and leads to achievement of tangible, sustainable program results. The CCSS is the primary provider of health care in the country. The PforR “Strengthening Universal Health Insurance in Costa Rica" was approved by the World Bank's Board of Executive Directors in 2016, with the aim of improving the availability and quality of the universal health insurance system while boosting the institutional efficiency of the CCSS. Through the PforR, the CCSS successfully undertook strategic and complex health sector reforms that have had significant impact on quality of care, equity, and efficiency in Costa Rica’s health sector. This series of knowledge reports, developed by the World Bank in collaboration with the CCSS, aims to document the drivers of success, how challenges were faced, and crucial lessons learned during the design and implementation of the PforR and its associated transformative reforms. The overarching objective is to provide a practical guide for other countries interested in implementing similar programs.

    Introducing Strategic Purchasing in Costa Rica

    The Costa Rican Social Security Fund (CCSS) has taken its first steps towards the introduction of strategic purchasing of healthcare services with a pilot that was introduced for the first time as part of the institution’s 2024 budget, which is currently being implemented. With this highly relevant reform, the CCSS begins its transition from a resource allocation model based solely on the historical budget to (i) a capitation model at the first level of care and (ii) the introduction of prospective elements at the hospital level. The purpose of the review in the resource allocation methodology is to align resources with the health needs of the population and the supply of services, while encouraging the achievement of specific strategic objectives, such as increasing productivity, accessibility, and quality of services.

    Supporting Youth at Risk

    The World Bank has produced this policy Toolkit in response to a growing demand from our government clients and partners for advice on how to create and implement effective policies for at-risk youth. The author has highlighted 22 policies (six core policies, nine promising policies, and seven general policies) that have been effective in addressing the following five key risk areas for young people around the world: (i) youth unemployment, underemployment, and lack of formal sector employment; (ii) early school leaving; (iii) risky sexual behavior leading to early childbearing and HIV/AIDS; (iv) crime and violence; and (v) substance abuse. The objective of this Toolkit is to serve as a practical guide for policy makers in middle-income countries as well as professionals working within the area of youth development on how to develop and implement an effective policy portfolio to foster healthy and positive youth development.

    The Republic of Yemen - Economic Growth : Sources, Constraints and Potentials

    High and sustained rate of economic growth in Yemen is a necesary, though not sufficient, condition for reduction of the high incidence of poverty and for raising the living standards of Yemeni citizens. Evidence in this report suggests that the main obstacle to rapid and sustained economic growth is the weak governance that characterizes Yemen in addition to the weaknesses in domestic security, property rights, and rule of law systems. Weak governance in Yemen is characterized/manifested by widespread corruption, lack of transparency and accountability, inefficiency in the interaction of public officials and private business, ineffective or absent market promoting institutions such as those enforcing contracts (courts, tribunals, etc.), poor performance of the public sector in terms of delivering essential goods and implementing programs, associated lack of incentives and skills in the civil service, and weak enforceability of contracts and rulings. Along with ordering the governance situation, there are areas that should receive government priority in the short and medium term, including: 1) enhancing domestic security to boost economic activity in all economic sectors; 2) removing excessive and arbitrary regulations to strengthen basic infrastructure and other services and to attract private investment into these sectors; 3) legal and judicial reform; and 4) sustained implementation of civil service reforms.

    Countries invest in basic education to provide their citizens with the means to acquire the foundations for building human capital. Countries with good school education systems provide equity of access, i.e., encourage all children, irrespective of gender, household income, and geographical location, from the early years onwards, to participate in the full cycle of education. Good education systems have learning environments that lead to strong learning outcomes. Since education is the basis for human capital development and improves individual productivity and earnings, good education systems contribute both to economic growth and social equity. This report looks at the role played by public expenditure in improving access, equity, quality, and learning in basic education in the Philippines. It builds on work undertaken earlier, especially the Basic Education Public Expenditure Review and the Philippines Public Education Expenditure Tracking and Quantitative Service Delivery Survey. Specifically, this review provides a comparative picture of sector performance, where possible, between the periods 2002 to 2008 and 2009 to 2017, the former being the period studied by BEPER (2012). Chapter one looks at quantity and quality in basic education, Chapter two examines equity issues, and chapter three looks at patterns of public expenditure in basic education. In the remaining section of this introduction, a brief description of how basic education is managed and financed in the Philippines is provided.

    Commodity Markets Outlook, October 2024

    Commodity prices are expected to decrease by 5 percent in 2025 and 2 percent in 2026. The projected declines are led by oil prices but tempered by price increases for natural gas and a stable outlook for metals and agricultural raw materials. The possibility of escalating conflict in the Middle East represents a substantial near-term upside risk to energy prices, with potential knock-on consequences for other commodities. However, over the forecast horizon, longer-term dynamics—including decelerating global oil demand, diversifying oil production, and ample oil supply capacity—suggest sizable downside risks to oil prices, especially if OPEC+ unwinds its latest production cuts. There are also dual risks to industrial commodity demand stemming from economic activity. On the one hand, concerted stimulus in China and above-trend growth in the United States could push commodity prices higher. On the other, weaker-than-anticipated global industrial activity could dampen them. Following several overlapping global shocks in the early 2020s, which drove parallel swings in commodity prices, commodity markets appear to be departing from a period of tight synchronization. A Special Focus analyzes commodity price synchronization over time and considers the relative importance across commodity cycles of a wide range of demand and supply shocks, including global demand shocks and shocks specific to different commodity markets. It concludes that, while supply shocks were the dominant commodity price driver in the early 2000s and around the global financial crisis, post-pandemic price movements have been more substantially shaped by commodity-specific shocks, such as those related to conflicts.
