External Assessment of Quality of Care in the Health Sector in Colombia

Abstract
This report presents findings from an assessment of quality of care in the health sector in Colombia, using a novel methodology developed by the World Bank Group to assess government oversight, promotion and stewardship of quality of care in the health sector. This assessment of quality of care in the health sector in Colombia examines how well governments strategize, plan and measure quality; set standards of care; build capacity for quality improvement in the sector; ensure adequate resources are available and well distributed to support quality results; hold organizations accountable for quality results; apply quality policies consistently; and achieve superior quality of care results. This methodology evaluates these eight domains using 49 criteria based on 171 standards. The assessment is based on quantitative analytics, key informant interviews, over 30 visits of public and private healthcare facilities, a review of key policy documents and a survey of a sample of health insurance companies. The assessment is a joint initiative from the World Bank and the International Finance Corporation (IFC) and was undertaken for the first time in Colombia. It took place from January to June 2019.

    Arab Republic of Egypt : Management and Service Quality in Primary Health Care Facilities in the Alexandria and Menoufia Governorates

    This report provides an assessment of the performance of public primary facilities in the Alexandria and Menoufia governorates. The performance is evaluated against the standards introduced with the Health Sector Reform Program; analyzes the quality perceptions, health situation, utilization and economic situation of households living in the catchment areas of the facilities; and examines the management processes of different institutions involved in primary care. Despite Egypt's health sector reform efforts, evidence suggests that issues remain in the quality of service and management in both reformed and non-reformed public primary care facilities, including availability of supplies, correct co-payment exemptions for the poor, and consequently, utilization through the population. There is also increasing evidence that the demand-side empowerment of beneficiaries could improve the governance of health care, which would lead to a quality increase and higher utilization of health care. This suggests the need to explore the potential for demand-side mechanisms to improve service delivery and help ensure improvements in individual and population health.

    Making Services Work : Indicators, Assessments, and Benchmarking of the Quality and Governance of Public Service Delivery in the Human Development Sectors

    Improving governance is central to improving results in human development. It is clear that money is not enough: improved outcomes from service delivery require better governance, including mechanisms for holding service providers accountable and appropriate incentives for performance. There is therefore a growing demand for indicators to measure how and whether these processes work, and how they affect health and education results. This paper makes the case for measuring governance policies and performance, and the quality of service delivery in health and education. It develops a framework for selecting and measuring a set of indicators and proposes options, drawing from new and innovative measurement tools and approaches. The paper proposes the adoption of a more systematic approach that will both facilitate the work of health and education policymakers and allow for cross-country comparisons and benchmarking.

    Healthy Partnerships : How Governments Can Engage the Private Sector to Improve Health in Africa

    Health systems across Africa are in urgent need of improvement. The public sector should not be expected to shoulder the burden of directly providing the needed services alone, nor can it, given the current realities of African health systems. Therefore to achieve necessary improvements, governments will need to rely more heavily on the private health sector. Indeed, private providers already play a significant role in the health sector in Africa and are expected to continue to play a key role, and private providers serve all income levels across sub- Saharan Africa's health systems. The World Health Organization (WHO) and others have identified improvements in the way governments interact with and make use of their private health sectors as one of the key ingredients to health systems improvements. Across the African region, many ministries of health are actively seeking to increase the contributions of the private health sector. However, relatively little is known about the details of engagement; that is, the roles and responsibilities of the players, and what works and what does not. A better understanding of the ways that governments and the private health sector work together and can work together more effectively is needed. This Report assesses and compares the ways in which African governments are engaging with their private health sectors. Engagement is defined, for the purposes of this report, to mean the deliberate, systematic collaboration of the government and the private health sector according to national health priorities, beyond individual interventions and programs. With effective engagement, one of the main constraints to better private sector contributions can be addressed, which in turn should improve the performance of health systems overall.

    Egypt : Health Sector Reform and Financing Review

    In 1997, the Ministry of Health and Population (MOHP) of the Government of Egypt (GOE) launched a comprehensive Health Sector Reform Program (HSRP) aiming to develop a national health system, based on social insurance that would address existing problems in equity, access, efficiency, quality and financial sustainability. The purpose of the Health Sector Reform and Financing Review is to provide the GOE and its development partners with a critical assessment of experience to date with the reform. At a critical juncture of the program's life, such an assessment is imperative to inform future phases of the reform and to revalidate, modify, or if necessary revisit the model envisioned at the outset. This study is comprised of five sections: the first sets the background for the study by reassessing the health sector problems that triggered the reform efforts; in the second section, the status of implementation of the HSR Pilot Project will be reviewed in relation to the model envisioned at the outset; section four tries to interpret why implementation of the HSR Pilot Project met, exceeded, or fell short of expectations; in section four, short-term and medium-term reform strategies are proposed aimed at expanding and sustaining the HSR Pilot Project; finally section five concludes the study with a summary of main findings and presents a road map for the future of health sector reform in Egypt.

    Romania : Health Sector Policy Note

    Many of Romania's basic health indicators have shown steady improvement since the 1970s. Both male and female life expectancy has gone up, and infant and maternal mortality have declined. Despite this progress, Romania still faces considerable challenges in improving the overall health status of its population. The overarching challenge for the next Romanian government is how to improve equity in access to better quality health care services in a financially sustainable manner. To achieve this goal, the government will need to consider not only overall funding levels, but also further changes to the composition of health sector funding, the distribution and organization of service delivery, accountability and decisions regarding who pays for what services, and incentive mechanisms to improve service quality. The likelihood of an economic slowdown and corresponding pressure on government budgets means that containing health care costs while simultaneously seeking ways to improve the equity, quality and efficiency of care will be paramount. The report ends with several policy options or recommendations.

    Empowerment in Practice : From Analysis to Implementation

    This book represents an effort to present an easily accessible framework to readers, especially those for whom empowerment remains a puzzling development concern, conceptually and in application. The book is divided into two parts. Part 1 explains how the empowerment framework can be used for understanding, measuring, monitoring, and operationalizing empowerment policy and practice. Part 2 presents summaries of each of the five country studies, using them to discuss how the empowerment framework can be applied in very different country and sector contexts and what lessons can be learned from these test cases. While this book can offer only a limited empirical basis for the positive association between empowerment and development outcomes, it does add to the body of work supporting the existence of such a relationship. Perhaps more importantly, it also provides a framework for future research to test the association and to prioritize practical interventions seeking to empower individuals and groups.

    Too often, services fail poor people in access, in quality, and in affordability. But the fact that there are striking examples where basic services such as water, sanitation, health, education, and electricity do work for poor people means that governments and citizens can do a better job of providing them. Learning from success and understanding the sources of failure, this year’s World Development Report, argues that services can be improved by putting poor people at the center of service provision. How? By enabling the poor to monitor and discipline service providers, by amplifying their voice in policymaking, and by strengthening the incentives for providers to serve the poor. Freedom from illness and freedom from illiteracy are two of the most important ways poor people can escape from poverty. To achieve these goals, economic growth and financial resources are of course necessary, but they are not enough. The World Development Report provides a practical framework for making the services that contribute to human development work for poor people. With this framework, citizens, governments, and donors can take action and accelerate progress toward the common objective of poverty reduction, as specified in the Millennium Development Goals.

    Human Resources for Mental Health Service Delivery in Viet Nam

    "Human Resources for Mental Health Service Delivery in Viet Nam" provides an overview of the country’s current state of and challenges to mental health service delivery. The framework of the report is composed of four interconnected domains: health care, social services, education and mental health literacy, and informal care systems. The organizational structure, significant achievements, critical gaps, and problems in mental health service delivery at the institutional and community levels are highlighted in terms of public demand, availability, accessibility, and quality of service. The report uses new empirical data from surveys, workshops, and group discussions with key stakeholders. It describes the mental health workforce in Viet Nam and analyzes critical issues, including the shortage of professionals (psychiatrists, mental health nurses, psychologists, psychotherapists, social workers, occupational therapists, and others). Given the need to develop all levels of mental health care, the report addresses the uneven distribution of the provision of service between levels of health care institutions and rural and urban regions, competency mismatches, job satisfaction, recruitment, and challenges to the retention of mental health workers. The report also examines the need for mental health education and training at the institutional and structured program levels, as well as the supply constraints to the future development of the mental health workforce. The interdisciplinary team of authors emphasizes the urgent need for Viet Nam to strengthen its human resources for mental health service delivery toward achieving universal health coverage, including all mental disorders. The report’s evidence-based recommendations include multisectoral workforce planning; transformation of education and training; coordination, integration, and retention of the available workforce; improvement of the workforce governance framework; and strengthened mental health financing.

    Poverty and Shared Prosperity 2022

    Poverty and Shared Prosperity 2022: Correcting Course provides the first comprehensive analysis of the pandemic’s toll on poverty in developing countries. It identifies how governments can optimize fiscal policy to help correct course. Fiscal policies offset the impact of COVID-19 on poverty in many high-income countries, but those policies offset barely one quarter of the pandemic’s impact in low-income countries and lower-middle-income countries. Improving support to households as crises continue will require reorienting protective spending away from generally regressive and inefficient subsidies and toward a direct transfer support system—a first key priority. Reorienting fiscal spending toward supporting growth is a second key priority identified by the report. Some of the highest-value public spending often pays out decades later. Amid crises, it is difficult to protect such investments, but it is essential to do so. Finally, it is not enough just to spend wisely - when additional revenue does need to be mobilized, it must be done in a way that minimizes reductions in poor people’s incomes. The report highlights how exploring underused forms of progressive taxation and increasing the efficiency of tax collection can help in this regard. Poverty and Shared Prosperity is a biennial series that reports on global trends in poverty and shared prosperity. Each report also explores a central challenge to poverty reduction and boosting shared prosperity, assessing what works well and what does not in different settings.
