On SARS Type Economic Effects During Infectious Disease Outbreaks

Abstract
Infectious disease outbreaks can exact a high human and economic cost through illness and death. But, as with severe acute respiratory syndrome (SARS) in East Asia in 2003, or the plague outbreak in Surat, India, in 1994, they can also create severe economic disruptions even when there is, ultimately, relatively little illness or death. Such disruptions are commonly the result of uncoordinated and panicky efforts by individuals to avoid becoming infected, of preventive activity. This paper places these "SARS type" effects in the context of research on economic epidemiology, in which behavioral responses to disease risk have both economic and epidemiological consequences. The paper looks in particular at how people form subjective probability judgments about disease risk. Public opinion surveys during the SARS outbreak provide suggestive evidence that people did indeed at times hold excessively high perceptions of the risk of becoming infected, or, if infected, of dying from the disease. The paper discusses research in behavioral economics and the theory of information cascades that may shed light on the origin of such biases. The authors consider whether public information strategies can help reduce unwarranted panic. A preliminary question is why governments often seem to have strong incentives to conceal information about infectious disease outbreaks. The paper reviews recent game-theoretic analysis that clarifies government incentives. An important finding is that government incentives to conceal decline the more numerous are non-official sources of information about a possible disease outbreak. The findings suggest that honesty may indeed be the best public policy under modern conditions of easy mass global communications.

    Climate and Social Sustainability in Fragility, Conflict, and Violence Contexts

    Climate change is widely recognized as a driver of violent conflict, but its broader social effects remain less understood. Ignoring these dimensions risks a vicious cycle where climate policies might undermine socially just adaptation. Evidence is still limited on how climate shocks influence political participation, trust, or migration. This paper helps fill that gap by examining links between climate change, conflict, and social sustainability, with a focus on inclusion, resilience, cohesion, and legitimacy. Using secondary data from 2019–24, the study applies simple correlation-based methods to test three hypotheses on the nature, severity, and composition of these associations. The analysis combines multiple climate impact measures, new conflict classifications, recent social sustainability frameworks, and controls for population and geography. The results reveal strong correlations—not causation—between climate events and contexts of fragility, conflict, and violence. Climate impacts are most pronounced in both national and subnational conflict settings. The study also finds robust links between fragility, conflict, and violence and low levels of social sustainability, reflecting its role as both a driver and consequence of conflict. Some dimensions—such as violent events and insecurity—appear weaker in areas most affected by climate shocks. Two of the hypotheses are supported, and one remains inconclusive.

    The Macroeconomic Implications of Climate Change Impacts and Adaptation Options

    Estimating the macroeconomic implications of climate change impacts and adaptation options is a topic of intense research. This paper presents a framework in the World Bank's macrostructural model to assess climate-related damages. This approach has been used in many Country Climate and Development Reports, a World Bank diagnostic that identifies priorities to ensure continued development in spite of climate change and climate policy objectives. The methodology captures a set of impact channels through which climate change affects the economy by (1) connecting a set of biophysical models to the macroeconomic model and (2) exploring a set of development and climate scenarios. The paper summarizes the results for five countries, highlighting the sources and magnitudes of their vulnerability --- with estimated gross domestic product losses in 2050 exceeding 10 percent of gross domestic product in some countries and scenarios, although only a small set of impact channels is included. The paper also presents estimates of the macroeconomic gains from sector-level adaptation interventions, considering their upfront costs and avoided climate impacts and finding significant net gross domestic product gains from adaptation opportunities identified in the Country Climate and Development Reports. Finally, the paper discusses the limits of current modeling approaches, and their complementarity with empirical approaches based on historical data series. The integrated modeling approach proposed in this paper can inform policymakers as they make proactive decisions on climate change adaptation and resilience.

    Institutional Capacity for Policy Implementation: An Analytical Framework

    State capacity is an important prerequisite for policy implementation, yet at the country level it is difficult to measure, assess, and reform. This paper proposes a focus on institutional capacity: the ability of public institutions to implement the specific policy mandates for which they are responsible. Based on a review of existing literature, the paper defines the different dimensions that compose institutional capacity and groups them into two cross-cutting categories: organizational dimensions (personnel, financial resources, information systems, and management practices) and governance dimensions (transparency, independence, and accountability). The paper proposes measures for organizational and governance dimensions using existing data, shows intra-institutional variation of these measures within countries, and discusses how new data could be collected for better measurement of these concepts. Finally, the paper illustrates how the framework can be used to diagnose the sources of common problems related to weak policy implementation.

    South Africa’s Fragmented Cities: The Unequal Burden of Labor Market Frictions

    Using high-resolution administrative, census, and satellite data, this paper shows that South African cities are characterized by spatial mismatches between where people live and where jobs are located, relative to 20 global peers. Areas within 5 kilometers of commercial centers have 9,300 fewer residents per square kilometer than expected, which is 60 percent below the global median. Poor, dense neighborhoods are most affected. In Johannesburg, a 10-percentile increase in distance from the nearest business hub corresponds to a 3.7-percentile drop in asset wealth (a proxy of household wellbeing) and 4.9-percentile drop in employment. In Cape Town, the declines are 4.0 and 3.7 percentiles, respectively. Employment is 87 percent lower in the poorest decile than the richest in Johannesburg and 61 percent lower in Cape Town. These findings suggest that South Africa’s spatial organization of people and economic activity constrains agglomeration and reinforces inequality. This methodology provides a scalable and standardized data-driven framework to analyze spatial accessibility and agglomeration frictions in complex, data-constrained urban systems.

    Investment in Emerging and Developing Economies

    The world faces a pressing challenge to meet key development objectives amid slowing growth and rising macroeconomic and geopolitical risks. With the number of job seekers rising rapidly, infrastructure shortfalls continuing to be large, and climate costs mounting, the case for a significant investment push has never been stronger. Yet the capacity to respond in many emerging markets and developing economies has eroded. Since the global financial crisis, investment growth has slowed to about half its pace in the 2000s, with both public and private investment weakening. Foreign direct investment inflows—a critical source of capital, technology, and managerial know-how—have also fallen sharply and become increasingly concentrated, leaving low-income countries with only a marginal share. The risks of further retrenchment are significant, as trade tensions, policy uncertainty, and elevated debt levels continue to weigh on investment. Reigniting momentum will require ambitious domestic reforms to strengthen institutions, rebuild macro-fiscal stability, and deepen trade and investment integration—the foundations of a supportive business climate. At the same time, international cooperation is indispensable. A renewed commitment to a predictable system of cross-border trade and investment flows, combined with scaled-up financial support and sustained technical assistance, is essential to help emerging markets and developing economies—especially low-income countries and economies in fragile and conflict situations—bridge financing gaps and implement the domestic reforms needed to restore investment as an engine of growth, jobs, and development.

    Public Health Surveillance Toolkit : A Guide for Busy Task Managers

    This toolkit draws on the expertise of public health practitioners who have experience with public health surveillance and who have recognized the core role of surveillance in public health. These practitioners have advocated for surveillance programs, supplied innovative ideas, and provided insightful critiques over many years. This toolkit also draws on the experience of Bank staff and technical experts from the PAHO and the CDC who have contributed to Bank missions. The toolkit also makes use of WHO references, primarily those from the WHO s Web site. Part A of this toolkit provides some theoretical concepts, and knowledge about surveillance that has been gained through applying these concepts and the practice of surveillance in developing countries. Part B provides information that will be useful to Task Managers as they prepare loans for strengthening public health surveillance systems. Several World Bank experiences are shared. The focus of part B is on practical aspects of surveillance and on lessons learned.

The report also discusses the health threat posed by alarmingly low levels of blood supplies, fostered by a culture that places little value on donating blood, public fear of being infected by giving blood, and the near absence of donor promotion campaigns.

    People, Pathogens and Our Planet : The Economics of One Health

    This study aims to build on the findings from the previously mentioned studies, and seeks to provide more detailed information on the costs of the various functions and categories of expenditure involved in the establishment and operation of system for the prevention and control of emerging zoonotic diseases at country and global level. It will also seek to provide information on efficiency and effectiveness gains that will result from the introduction of a One Health approach. With these aims, the study has two target audiences: (a) project planners, who will benefit from the information of the costs of setting up surveillance and control systems to be used as benchmarks when planning preparedness and control operations; and (b) policy planners at the decision-making level, who would use the information on the efficiency and effectiveness gains to guide them in the decision-making process regarding the eventual introduction of One Health. This report disaggregates costs by task, making explicit those activities that are critical for effectiveness and identifying scope for efficiencies. The analysis draws on a range of data sources and earlier work, including integrated national action plans for, and World Bank staff appraisal reports on, avian and pandemic influenzas responses, a survey of the directors of wildlife services, assessments of veterinary systems in developing countries, and OIE (Office International des Epizooties - World Organization for Animal Health) analyses of disease prevention systems.

    When Institutions Work

    Nigeria is a country of immense natural resources and potential, but the government’s capacity to deliver public goods has generally been weak. It was against this backdrop that Nigeria faced the arrival within its borders of the deadly Ebola virus disease in July 2014. Despite assurances that the Nigerian government was prepared to respond to an outbreak of Ebola, the country was caught unaware and forced to mount an emergency response. Yet despite these serious concerns, the spread of Ebola was successfully contained in Nigeria. This case study seeks to understand why Nigeria’s Ebola response was so successful despite the challenging context. The case study will focus on institutional architecture and political will, taking an exploratory qualitative approach to examine the institutional dynamics and motivations among various stakeholders involved in the country’s response. The aim is to distill lessons that may be applied to other emergency response initiatives, as well as elsewhere in the health sector and in other areas of service delivery. A proactive communication strategy is required to build a broader coalition of support, and demand-side actors such as nongovernmental organizations play a helpful role.

    Uruguay - Strengthening Participatory Monitoring and Evaluation of Social Policy : Nonlending Technical Assistance Project, Report on Phase 2

    The objectives of the Uruguay Nonlending Technical Assistance Project (UY NLTA) were to: (i) evaluate the impact of providing a safety net program's (PANES's) social emergency programs; (ii) inform a dialogue about the future role of social programs and of MIDES as a lead authority responsible for the coordination and oversight of social policy in Uruguay; and (iii) strengthen Ministry of Social Development's (MIDES's) capacity to assess, monitor, and evaluate social policy in Uruguay, with considerable weight given to participatory approaches to monitoring and evaluation. Expected outcomes of the project included: (a) increased efficiency in the implementation of Uruguay's social programs through the consolidation of existing and dispersed databases and the strengthening of MIDES's Monitoring and Evaluation (M&E) system; (b) increased transparency and strengthened accountability in selected departments through the piloting of a participatory monitoring and evaluation (PM&E) approach; (c) stronger engagement of civil society in public sector reform in Uruguay; and (d) informed decisions about the future of social programs under the Plan Nacional de Equidad, and the role of MIDES, on the basis of the evaluations of PANES. The rest of this report is organized as follows: section two presents the PSIA's main contribution by laying out the main results described in the technical note. Section three presents the Technical Assistance (TA) contribution and is divided into three subsections. Subsection one looks at the contributions to strengthen the capacity within MIDES to conduct impact analyses; subsection two analyzes activities in the TA aimed at making the M&E system operational; and subsection three describes the International Workshop on M&E. Section four describes the risks and main problems faced during the implementation of phase two and includes lessons learned, pending agenda, and next steps.

    Stimulating Business and Employment in Yemen

    Yemen is at a crossroads. Two years after popular unrest led to the ousting of the former president, there is now a coalition government, and the beginnings of a national political transition. March 2013 saw the launch of the National Dialogue Conference (NDC) that has brought together political and civil society actors from across the spectrum to formulate a constitution and bring closure not only to the unrest that began in 2011, but to decades of unresolved grievances and conflicts within Yemen. Expectations are high and much is riding on the outcome of the NDC, not least of which is the future direction of the Yemeni economy. As in most developing countries, the Yemeni private sector consists predominantly of micro, small, and medium enterprises (MSMEs) - over 97 presentage of firms (about 290,000 in number) have less than 25 employees with total estimated employment of over 600,000 workers, including approximately 30,000 women. The inclusion of these MSMEs in Yemen's economic recovery is critical for social stability as they represent a significant portion of the jobs outside the public sector and a significant source of income for segments of the Yemeni population. Even more crucially, they also represent an important source of potential future job growth.

    Water Supply, Sanitation, and Hygiene (WASH) and COVID-19

    Safely managed water supply, sanitation, and hygiene (WASH) services are an essential part of preventing disease and protecting human health during infectious disease outbreaks, including the current COVID-19 pandemic. One of the most cost-effective strategies for increasing pandemic preparedness, especially in resource-constrained settings, consists of investing to strengthen core public health infrastructure, including water supply and sanitation systems. Good and consistently applied WASH and waste management practices serve as essential barriers to human-to-human transmission of the COVID-19 virus in communities, homes, health care facilities, schools, and other public spaces.

    Taxes, Spending, and Equity: International Patterns and Lessons for Developing Countries

    Taxes and public spending underpin the basic administration of government and finance the human capital and infrastructure investments needed for economic growth. They can also have a significant and immediate impact on poverty and inequality. The question of how public finance can support longer-term growth objectives while promoting equity has become even more important in recent years, given the high fiscal deficits and debt levels most countries emerged with in the aftermath of the COVID-19 pandemic. These included the increasing cost of debt and the need to restart environmentally sustainable growth while helping households address the learning losses and other social scars caused by the pandemic. This paper examines the global evidence on which households pay which taxes and who benefits from what spending, and critically, the net effect on different households across the income distribution. The aim is to identify the patterns and lessons that emerge for designing progressive fiscal policies. A global dataset of 96 countries is assembled, spanning all regions of the world and all national income levels, grounded in the Commitment to Equity (CEQ) approach to fiscal incidence.

    Digital Africa

    All African countries need better and more jobs for their growing populations. "Digital Africa: Technological Transformation for Jobs" shows that broader use of productivity-enhancing, digital technologies by enterprises and households is imperative to generate such jobs, including for lower-skilled people. At the same time, it can support not only countries’ short-term objective of postpandemic economic recovery but also their vision of economic transformation with more inclusive growth. These outcomes are not automatic, however. Mobile internet availability has increased throughout the continent in recent years, but Africa’s uptake gap is the highest in the world. Areas with at least 3G mobile internet service now cover 84 percent of Africa’s population, but only 22 percent uses such services. And the average African business lags in the use of smartphones and computers as well as more sophisticated digital technologies that catalyze further productivity gains. Two issues explain the usage gap: affordability of these new technologies and willingness to use them. For the 40 percent of Africans below the extreme poverty line, mobile data plans alone would cost one-third of their incomes—in addition to the price of access devices, apps, and electricity. Data plans for small- and medium-size businesses are also more expensive than in other regions. Moreover, shortcomings in the quality of internet services—and in the supply of attractive, skills-appropriate apps that promote entrepreneurship and raise earnings—dampen people’s willingness to use them. For those countries already using these technologies, the development payoffs are significant. New empirical studies for this report add to the rapidly growing evidence that mobile internet availability directly raises enterprise productivity, increases jobs, and reduces poverty throughout Africa. To realize these and other benefits more widely, Africa’s countries must implement complementary and mutually reinforcing policies to strengthen both consumers’ ability to pay and willingness to use digital technologies. These interventions must prioritize productive use to generate large numbers of inclusive jobs in a region poised to benefit from a massive, youthful workforce—one projected to become the world’s largest by the end of this century.
