Abstract
The health equity and financial protection reports are short country-specific volumes that provide a picture of equity and financial protection in the health sectors of low-and middle-income countries. Topics covered include: inequalities in health outcomes, health behavior and health care utilization; benefit incidence analysis; financial protection; and the progressivity of health care financing. Kenya's government is committed to improving equity and financial protection in health by implementing the Second National Health Sector Strategic Plan (NHSSP II). Kenya spends 4.3 per cent (2009) of its gross domestic product (GDP) on health. This is lower than the average spending levels in other lower income countries in Africa, which spent an average of 6.5 per cent (2009) of their GDP on health. The functions of the health system in Kenya have historically been centralized through top-down decision-making and resource allocations. However, in the past decade Kenya has committed to decentralization of certain core functions to the district level. These include managing the health management system, making resource allocation decisions, and delivering health services. The central government maintains control over the majority of the key functions of the health system including staffing, contracting, and maintaining the national health information system. Kenya has a form of social insurance through the 40 year-old National Hospital Insurance Fund (NHIF). Employees in the formal sector are compulsorily insured and must make monthly contributions from their wages.

Pakistan's government is committed to improving the equity of health outcomes and the ability to offer financial protection in the health sector through the implementation of the National Health Policy. Pakistan spends 2.62 per cent (2009) of its gross domestic product (GDP) on health. This is far lower than the average spending levels in other countries in the South Asia Region, which have spent an average of 5.3 per cent (2009) of their GDP on health.Publication Health Equity and Financial Protection in Zambia(Washington, DC, 2012-05-21)This report analyzes equity and financial protection in the health sector of Zambia. In particular, it examines inequalities in health outcomes, health behavior and health care utilization; benefit incidence analysis; financial protection; and the progressivity of health care financing. Data are drawn from the 2007 Zambia demographic and health survey, the 2006 Zambia living conditions monitoring survey, the 2003 Zambia world health survey and the 2003 Zambia national health accounts. All analyses are conducted using original survey data and employ the health modules of the ADePT software. Overall, health care financing in Zambia in 2006 was fairly progressive, i.e. the better-off spent a larger fraction of their consumption on health care than the poor. The financing sources that contribute to the overall progressivity of health care finance are general taxation, which finances 42 per cent of domestic spending on health, and contributions made by private employers, which finance 9 per cent of spending. An additional contribution to overall progressivity is made through pre-payment mechanisms, but this remains fairly limited given that they only represent 1 per cent of total health finance. Out-of-pocket health payments, which account for 47 per cent of total health financing, appear to be proportional to income, with only slight and not statistically significant evidence of progressivity.Publication Health Equity and Financial Protection in Ghana(Washington, DC, 2012-05-21)The health equity and financial protection reports are short country-specific volumes that provide a picture of equity and financial protection in the health sectors of low-and middle-income countries. Ghana's government is committed to improving equity and financial protection in the health sector. In 2005, the Government of Ghana amended its growth and poverty reduction strategy report to include a new target in the country's development: to reach middle income status by the year 2015 (Republic of Ghana 2005). Ghana's Minister of health has called attention to the role that health plays in economic development and has placed equity in both access and delivery of health services as a top priority for reaching middle income status (Ministry of health 2007). Ghana spends 8.1 per cent (2009) of its gross domestic product (GDP) on health. This is greater than the spending levels in other lower middle-income countries in Africa, which spend an average of 5.8 per cent (2009) of their GDP on health. Ghana provides free health services for certain vulnerable groups, such as children under five, people over 70, and pregnant women. In addition, immunization and services to combat certain communicable diseases are provided free of charge.Publication Health Equity and Financial Protection in Malawi(Washington, DC, 2012-05-17)The health equity and financial protection reports are short country-specific volumes that provide a picture of equity and financial protection in the health sectors of low-and middle-income countries. Topics covered include: inequalities in health outcomes, health behavior and health care utilization; benefit incidence analysis; financial protection; and the progressivity of health care financing. Malawi's government is committed to improving equity and financial protection in the health sector. Equity is explicitly mentioned as one of the four objectives in the Health Sector Strategic Plan (HSSP). The overall objective of the HSSP is to contribute towards Malawi's attainment of the health and related millennium development goals. The specific objectives of the HSSP are, therefore, to: 1) increase coverage of the high quality Essential Health Package (EHP) services; 2) reduce risk factors to health; 3) improve equity and efficiency in the delivery of quality EHP services; and 4) strengthen the performance of the health system to support delivery of EHP services. Malawi spends 6.2 per cent (2009) of its gross domestic product (GDP) on health. This is similar to the average spending in other lower income countries in Africa, which have spent an average of 6.5 per cent (2009) of their GDP on health. Publication Sourcebook on the Foundations of Social Protection Delivery Systems(Washington, DC: World Bank, 2020-07-30)The Sourcebook synthesizes real-world experiences and lessons learned of social protection delivery systems from around the world, with a particular focus on social and labor benefits and services. It takes a practical approach, seeking to address concrete “how-to” questions, including: How do countries deliver social protection benefits and services? How do they do so effectively and efficiently? How do they ensure dynamic inclusion, especially for the most vulnerable and needy? How do they promote better coordination and integration—not only among social protection programs but also programs in other parts of government? How can they meet the needs of their intended populations and provide a better client experience? The Sourcebook structures itself around eight key principles that can frame the delivery systems mindset: (1) delivery systems evolve over time, do so in a non-linear fashion, and are affected by the starting point(s); (2) additional efforts should be made to “do simple well”, and to do so from the start rather than trying to remedy by after-the-fact adding-on of features or aspects; (3) quality implementation matters, and weaknesses in the design or structure of any core system element will negatively impact delivery; (4) defining the “first mile” for people interface greatly affects the system and overall delivery, and is most improved when that “first mile” is understood as the weakest link in delivery systems); (5) delivery systems do not operate in a vacuum and thus should not be developed in silos; (6) delivery systems can contribute more broadly to government’s ability to intervene in other sectors, such as health insurance subsidies, scholarships, social energy tariffs, housing benefits, and legal services; (7) there is no single blueprint for delivery systems, but there are commonalities and those common elements constitute the core of the delivery systems framework; (8) inclusion and coordination are pervasive and perennial dual challenges, and they contribute to the objectives of effectiveness and efficiency.Publication Kenya Country Climate and Development Report(Washington, DC: World Bank, 2023-11-16)The Kenya Country Climate and Development Report (CCDR) aims to identify the impact of climate change on Kenya’s economy. Through robust and rigorous analyses that cover climate impact modeling across multiple scenarios and the overall economy, sectoral issues, investment needs and potential sources of financing, the CCDR aims to identify high impact intervention areas that would support climate positive development. Action against climate change is imperative to avoid setting back Kenya’s aspiration of being an upper-middle-income country and reducing poverty in the next decade. In a business-as-usual scenario, inaction under different climate futures could dampen real GDP by 1.25 to 2.4 percent by 2030 and 3.61 to 7.25 percent by 2050 compared to the baseline. Climate impacts Kenya’s human, natural and physical capital and the impacts vary by region. By 2050, no climate action could also result in 1.1 million additional poor compared to the baseline under the pessimistic climate scenario, with communities in the arid and semi-arid areas being most hard-hit. Kenya can also be a key player in the global climate solutions arena if it maintains a low-carbon growth path. Kenya stands out among African and lower-middle-income countries due to its well-diversified and primarily low-carbon energy mix, with about 90 percent of electricity generation coming from renewable resources. Kenya could also generate carbon offsets through large-scale landscape restoration. The CCDR identifies five key action areas that could enable Kenya to meet its growth aspirations in an inclusive and climate-resilient manner. The three multisectoral action areas are: managing water, land, and forest for climate-resilient agriculture and rural economies; delivering people-centered resilience with climate-informed basic services and urbanization; and strengthening Kenya’s competitiveness in international markets through shifts in energy, transport, and digital systems. It is necessary to complement these the three action areas with two crosscutting actions areas - improving integration and coordination of climate action in policy, planning, and investment decision-making across the economy, and developing and operationalizing policy measures for mobilizing climate finance from private and public sector. Implementing these action areas should account for regional differences to climate risk exposure.Publication World Development Report 2006(Washington, DC, 2005)This year’s Word Development Report (WDR), the twenty-eighth, looks at the role of equity in the development process. It defines equity in terms of two basic principles. The first is equal opportunities: that a person’s chances in life should be determined by his or her talents and efforts, rather than by pre-determined circumstances such as race, gender, social or family background. The second principle is the avoidance of extreme deprivation in outcomes, particularly in health, education and consumption levels. This principle thus includes the objective of poverty reduction. The report’s main message is that, in the long run, the pursuit of equity and the pursuit of economic prosperity are complementary. In addition to detailed chapters exploring these and related issues, the Report contains selected data from the World Development Indicators 2005‹an appendix of economic and social data for over 200 countries. This Report offers practical insights for policymakers, executives, scholars, and all those with an interest in economic development.Publication Classroom Assessment to Support Foundational Literacy(Washington, DC: World Bank, 2025-03-21)This document focuses primarily on how classroom assessment activities can measure students’ literacy skills as they progress along a learning trajectory towards reading fluently and with comprehension by the end of primary school grades. The document addresses considerations regarding the design and implementation of early grade reading classroom assessment, provides examples of assessment activities from a variety of countries and contexts, and discusses the importance of incorporating classroom assessment practices into teacher training and professional development opportunities for teachers. The structure of the document is as follows. The first section presents definitions and addresses basic questions on classroom assessment. Section 2 covers the intersection between assessment and early grade reading by discussing how learning assessment can measure early grade reading skills following the reading learning trajectory. Section 3 compares some of the most common early grade literacy assessment tools with respect to the early grade reading skills and developmental phases. Section 4 of the document addresses teacher training considerations in developing, scoring, and using early grade reading assessment. Additional issues in assessing reading skills in the classroom and using assessment results to improve teaching and learning are reviewed in section 5. Throughout the document, country cases are presented to demonstrate how assessment activities can be implemented in the classroom in different contexts.Publication Implementation Know-how Briefs to Support Countries to Prioritize, Connect and Scale for a Digital-in-Health Future(Washington, DC: World Bank, 2023-08-18)Technology and data are integral to daily life. As health systems face increasing demands to deliver new, more, better, and seamless services affordable to all people, data and technology are essential. With the potential and perils of innovations like artificial intelligence the future of health care is expected to be technology-embedded and data-linked. This shift involves expanding the focus from digitization of health data to integrating digital and health as one: Digital-in-Health. The World Bank’s report, Digital-in-Health: Unlocking the Value for Everyone, calls for a new digital-in-health approach where digital technology and data are infused into every aspect of health systems management and health service delivery for better health outcomes. The report proposes ten recommendations across three priority areas for governments to invest in: prioritize, connect and scale. The Implementation Know-How Briefs serve as practical guides for countries as they implement the ten recommendations. Every Implementation Know-How Brief provides practical information to start planning and implementing how to implement the recommendations. It also contains key terminologies for those not familiar with a particular topic, provides key questions to ask, and a general orientation as to typical issues in these sectors.
