From Population Lending to HNP Results : The Evolution of the World Bank's Strategies in Health, Nutrition and Population

Abstract
This paper reviews the evolution of the World Bank's strategies in the health, nutrition, and population (HNP) sector in relation to both internal and global events, as background for the forthcoming evaluation by the Independent Evaluation Group (IEG) of the Bank's support for HNP. It summarizes the objectives, priorities, and strategies in HNP as expressed in official documents and revealed implicitly by its lending and non-lending activities. Special emphasis is placed on analysis of the period since the release of the 1997 HNP Strategy, which has guided the sector over the last decade. However, the report also reviews the Bank's earlier experiences in HNP, so as to provide a context for understanding the World Bank's current strategies. Detailed timelines annexed to the paper help to put the Bank's strategies and actions in the context of the evolution of global HNP themes. It aimed to help client countries: (i) improve the HNP outcomes of the poor and protect the population from the impoverishing effects of illness, malnutrition, and high fertility; (ii) enhance the performance of health systems; and (iii) secure sustainable health financing. The sector sought to achieve greater impact through emphasizing strategic policy directions in country assistance strategies, underpinning lending with analysis and research, increasing selectivity, improving client services, and stronger monitoring and evaluation. Throughout the recent phases, the World Bank's objectives remained largely consistent: improving health outcomes, especially among the poor; strengthening health systems; and securing sustainable financing. This review of the Bank's objectives and strategies, particularly over the past decade, suggests four priority areas of investigation for an evaluation of the Bank's HNP support.

    This paper updates the 1997 World Bank Health, Nutrition, and Population Strategy to enhance Bank capacity so that it continues to contribute to this virtuous circle in light of the momentous changes of the past decade in the architecture of development assistance for health (DAH) and of persisting HNP challenges worldwide. This 2007 HNP Strategy outlines the Bank vision for improving its own capacity to respond globally and with a country focus to the urgent issues posed by these changes and challenges.

    Improving Health, Nutrition and Population Outcomes in Sub-Saharan Africa : The Role of the World Bank

    In recent years, the focus in many developing regions is increasingly moving away from communicable disease, where as Africa continues to face a high burden of communicable disease, malnutrition, and fertility. At the same time, the ways in which the World Bank operates are changing - increasingly countries define their own priorities, resources are provided through programmatic approaches, and the Bank is focusing more on sharing knowledge, policy advice, research and analysis. This book takes an in-depth look at health, nutrition, and population (HNP) challenges faced by Sub-Saharan Africa, particularly how disease, malnutrition, and high fertility affect poverty reduction. The authors contend that the World Bank has a comparative advantage in contributing within four broad areas: 1) macroeconomics and health, 2) multi-sectoral action for health, 3) strengthening health systems, and 4) financing service delivery. They also address the opportunities and challenges within these four areas and conclude with suggestions on how the Bank can better operate within the sector and work effectively with partners.

    Nutrition Beyond the Health Sector : A Profile of World Bank Lending in Nutrition from 2000 to 2006

    The World Bank report Repositioning Nutrition as Central to Development (2006) explicitly recommended improving nutrition by not only working through the health sector, but also in non-health sectors such as agriculture and education. This report provides descriptive and financial profiles of the Bank's recent portfolio in nutrition (from FY2000 to late FY2006) to note the extent to which the Bank has actually gone outside the health sector to work on nutrition. In this period, 41 projects were assigned a theme of "food and nutrition security." Just over half of them (22 out of 41) had nutrition-related components or activities. Of these 22 projects, half fell within the health sector, and half fell outside.

    Management welcomes the Independent Evaluation Group (IEG) report results and performance 2012 of the World Bank group (RAP) and its overall positive assessment of the World Bank group development effectiveness. Management appreciates that the report provides a balanced picture of the World Bank group activities and recognizes that all three institutions have taken important steps to strengthen results, monitoring, and reporting. The report is especially useful to management to prioritize the challenges in the context of the ongoing efforts to strengthen focus on results.     Improving Effectiveness and Outcomes for the Poor in Health, Nutrition, and Population : An Evaluation of World Bank Group Support since 1997

    This evaluation aims to inform the implementation of the most recent the World Bank and International Finance Corporation (IFC) health, nutrition, and population (HNP) strategies to enhance the effectiveness of future support. It covers the period since fiscal year 1997 and is based on desk reviews of the portfolio, background studies, and field visits. The evaluation of the HNP support of the World Bank focuses on the effectiveness of policy dialogue, analytic work, and lending at the country level, while that of IFC focuses on the performance of health investments and advisory services before and after its 2002 health strategy. The themes it covers are drawn from the two strategies and the approaches adopted by international donors in the past decade. Independent Evaluation Group (IEG) has previously evaluated several aspects of the Bank's HNP support. IFC's support for the health sector has never been fully evaluated. Many lessons have been learned over the past decade about the successes and pitfalls of support for health reform: First, the failure to assess fully the political economy of reform and to prepare a proactive plan to address it can considerably diminish prospects for success. Political risks, the interests of key stakeholders, and the risk of complexity- issues the evaluation case studies found to be critical are often neglected in risk analysis in project appraisal documents for health reform projects. Second, reforms based on careful prior analytic work hold a greater chance of success, but analytic work does not ensure success. Third, the sequencing of reforms can improve political feasibility, reduce complexity, ensure that adequate capacity is in place, and facilitate learning. When implementation is flagging, the Bank can help preserve reform momentum with complementary programmatic lending through the Ministry of Finance, as it did in Peru and the Kyrgyz Republic. Finally, monitoring and evaluation are critical in health reform projects-to demonstrate the impact of pilot reforms to garner political support, but also because many reforms cannot work without a well-functioning management information system.

    Early Child Development, From Measurement to Action : A priority for Growth and Equity

    The World Bank recently hosted a symposium on the priority of early child development (ECD) for economic growth and equity. The participants urged application of population-based tools and measures to assess the outcomes of children's early years and children's readiness for school. This study is derived from the symposium and is a valuable resource for policy makers, economists, donors, and investors, as well as researchers and practitioners in early child development. It summarizes the current neuroscience on early child development and major longitudinal studies, the rationale and urgency for greater investment, and countries' innovative funding strategies. The report consists of 15 chapters authored by ECD experts and leaders in the field. The chapters are grouped into five main parts relating to the: business imperative and societal benefits of ECD investments; lessons from evaluation of longitudinal ECD interventions; countries' experiences in monitoring ECD interventions; innovative approaches to countries' financing of ECD initiatives; and next steps on the ECD agenda for the next 5 years. A theme highlighted at the symposium and enlarged upon here is the urgent need for evidence- and population-based instruments and measures to monitor, evaluate, and compare ECD interventions over time and across settings.

    Changing the Face of the Waters : The Promise and Challenge of Sustainable Aquaculture

    This study provides strategic orientations and recommendations for Bank client countries and suggests approaches for the Bank's role in a rapidly changing industry with high economic potential. It identifies priorities and options for policy adjustments, catalytic investments, and entry points for the Bank and other investors to foster environmentally friendly, wealth-creating, and sustainable aquaculture. The objectives of the study are to inform and provide guidance on sustainable aquaculture to decision makers in the international development community and in client countries of international finance institutions. The study focuses on several critical issues and challenges: 1) Harnessing the contribution of aquaculture to economic development, including poverty alleviation and wealth creation, to employment and to food security and trade, particularly for least developed countries (LDCs); 2) Building environmentally sustainable aquaculture, including the role of aquaculture in the broader suite of environmental management measures; 3) Creating the enabling conditions for sustainable aquaculture, including the governance, policy, and regulatory frameworks, and identifying the roles of the public and private sectors; and 4) Developing and transferring human and institutional capacity in governance, technologies, and business models with special reference to the application of lessons from Asia to Sub-Saharan Africa and Latin America.

    Making Monitoring and Evaluation Systems Work : A Capacity Development Toolkit

    There are constant and growing pressures on governments and organizations around the world to be more responsive to demands from internal and external stakeholders for good governance, accountability and transparency, greater development effectiveness and delivery of tangible results. Governments, parliaments, citizens, the private sector, non-governmental organizations (NGOs), civil society, international organizations, and donors are all among stakeholders interested in better performance. As demands for greater accountability and results have grown, there is an accompanying need for useful and useable results-based monitoring and evaluation systems to support the management of policies, programs, and projects. Governments and other organizations have many different kinds of tracking systems as part of their management toolkits: good human resource systems, financial systems, and accountability systems. They also need good feedback systems. A results-based monitoring and evaluation (M&E) system is essentially such a feedback system; it is a management tool to measure and evaluate outcomes, providing information for governance and decision making. Many management systems have been missing a feedback component to enable them to track the consequences of actions. Building an M&E system gives decision-makers an additional management tool by providing feedback on performance as a basis for future improvement.

    Climate Change and Migration : Evidence from the Middle East and North Africa

    Climate change is a major source of concern in the Middle East and North Africa (MENA) region, and migration is often understood as one of several strategies used by households to respond to changes in climate and environmental conditions, including extreme weather events. This study focuses on the link between climate change and migration. Most micro-level studies measure climate change either by the incidences of extreme weather events or by variation in temperature or rainfall. A few studies have found that formal and informal institutions as well as policies also affect migration. Institutions that make government more responsive to households (for example through public spending) discourage both international and domestic migration in the aftermath of extreme weather events. Migration is often an option of last resort after vulnerable rural populations attempting to cope with new and challenging circumstances have exhausted other options such as eating less, selling assets, or removing children from school. This study is based in large part on new data collected in 2011 in Algeria, Egypt, Morocco, Syria, and the Republic of Yemen. The surveys were administered by in-country partners to a randomly selected set of 800 households per country. It is also important to emphasize that neither the household survey results nor the findings from the qualitative focus groups are meant to be representative of the five countries in which the work was carried, since only a few areas were surveyed in each country. This report is organized as follows: section one gives synthesis. Section two discusses household perceptions about climate change and extreme weather events. Section three focuses on migration as a coping mechanisms and income diversification strategy. Section four examines other coping and adaptation strategies. Section five discusses perceptions about government and community programs.
