What Can We Learn from Nutrition Impact Evaluations? Lessons from a Review of Interventions to Reduce Child Malnutrition in Developing Countries

Abstract
This paper reviews recent impact evaluations of interventions and programs to improve child anthropometric outcomes- height, weight, and birth weight-with an emphasis on both the findings and the limitations of the literature and on understanding what might happen in a non-research setting. It further reviews the experience and lessons from evaluations of the impact of the World Bank-supported programs on nutrition outcomes. Specifically, the review addresses the following four questions: 1) what can be said about the impact of different interventions on children's anthropometric outcomes? 2) How do these findings vary across settings and within target groups, and what accounts for this variability? 3) What is the evidence of the cost-effectiveness of these interventions? 4) What have been the lessons from implementing impact evaluations of Bank-supported programs with anthropometric impacts? Although many different dimensions of child nutrition could be explored, this report focuses on child anthropometric outcomes-weight, height, and birth weight. These are the most common nutrition outcome indicators in the literature and the ones most frequently monitored by national nutrition programs supported by the World Bank. Low weight for age (underweight) is also the indicator for one of the Millennium Development Goals.

    Malnutrition in Afghanistan : Scale, Scope, Causes, and Potential Response

    This book has the potential to contribute to a reversing of this trend, whereby activities in not only the health sector but also in other sectors relevant to nutrition will gain increased support and prominence in national development planning. South Asia has by far the largest number of malnourished women and children, and no other region of the world has higher rates of malnutrition. Malnutrition in childhood is the biggest contributor to child mortality; a third of child deaths have malnutrition as an underlying cause. For the surviving children, malnutrition has lifelong implications because it severely reduces a child's ability to learn and to grow to his or her full potential. Malnutrition thus leads to less productive adults and weaker national economic performance. Therefore, the impact of malnutrition on a society's productivity and well being and a nation's long-term development is hard to underestimate. For the South Asia region of the World Bank, malnutrition is a key development priority, and in the coming years, the Bank intends to enhance dramatically its response to this challenge. As a first step, a series of country assessments such as this one are being carried out. These assessments will be used to reinforce the dialogue with governments and other development partners to scale up an evidence-based response against malnutrition. To succeed, we will need to address the problem comprehensively, which will require engaging several sectors. This assessment of malnutrition in Afghanistan lays out the scale, scope, and causes of the problem. The assessment also indicates key elements of a potential response.

    Mainstreaming Nutrition in Poverty Reduction Strategy Papers : What Does It Take? A Review of the Early Experience

    This paper reviews 40 full poverty reduction strategy papers (PRSPs) with regard to whether these strategy papers (1) recognize under-nutrition as a development problem in the country, (2) whether they use nutrition information for poverty analysis, and (3) whether the PRSP includes specific nutrition activities (policies, strategies, and programs) to deal with the unique nutrition problems in each country. The review shows that three quarters of the PRSPs recognize that under-nutrition is a development problem that leads to loss of human capital and/or productivity. Also, many PRSPs, either explicitly or implicitly, include country nutrition profiles in their poverty analysis. Consequently, a majority of PRSPs include strategies and specific actions to mitigate the effects of malnutrition. However, there appears to be little prioritization or sequencing of proposed actions. More importantly, the strategies and actions included in PRSPs often do not reflect an appropriate response to the nature of the nutrition problem in the country. In a quarter of countries with macronutrient deficiencies and about 40% of countries with micronutrient deficiencies, the PRSPs fail to address these two problems. Moreover, tackling nutrition issues requires greater institutional capacity and budget allocations than currently seem to exist. Gross mismatches between the causes of malnutrition and responses to the nutrition problem inevitably lead to a lack of impact and a waste of resources, which will further contribute to the marginalization of nutrition in future PRSPs.

    Institutional Arrangements for Nutrition in India : An Assessment of Capacity

    The report consists of the following seven chapters. The present chapter provides the context and the rationale for the study, and outlines the methodology. Chapter two describes the nutrition system in detail and discusses the capacity issues related to the division of labor (DoL) between the two primary programs of the nutrition system and within the Integrated Child Development Services, or ICDS. Chapter three delves into the factors that constrain leadership capacity at all levels chapter four discusses the key capacity constraints related to HR that limit the effective delivery of nutrition services. Chapter five discusses the training system as a sub-system of HR and its capacity constraints, and Chapter six covers the capacity assessment pertaining to monitoring, evaluation and management of information in the nutrition system. Chapter seven discusses the capacity constraints related to General Management Practices, or GMPs in the ICDS, especially focusing on supervision, accountability and work practices and procedures.

    Costed Plan for Scaling Up Nutrition

    This paper estimates country-specific costs and benefits of scaling up key nutrition investments in Nigeria. Building on the methodology established in the global report scaling up nutrition: what will it cost? Authors first estimate the costs and benefits of a nationwide scale up of ten effective nutrition-specific interventions. This will require an annual public investment of $837 million and would yield enormous benefits: over 8.7 million DALYs and 183,000 lives would be saved annually, while more than 3 million cases of stunting among children under five will be averted. As it is unlikely that the Government of Nigeria or its partners will find the $837 million necessary to reach full national coverage, authors also consider five potential scale-up scenarios based on considerations of burden of stunting, potential for impact, resource requirements and capacity for implementation in Nigeria. Using cost-benefit analyses authors propose scale-up scenarios that represent a compromise between the need to move to full coverage and the constraints imposed by limited resources and capacities. This analysis takes an innovative approach to nutrition costing by not only estimating the costs and benefits of nutrition-specific interventions, but also exploring costs for a selected number of nutrition-sensitive interventions implemented outside of the health sector. We identify and cost four candidate nutrition-sensitive interventions with impact potential in Nigeria, including bio-fortification of cassava, aflatoxin control, school-based deworming, and school-based promotion of good hygiene. Overall, these findings point to a candidate list of nutrition-sensitive approaches that represent a cost-effective approach to reducing child malnutrition in Nigeria. Moving forward, these results are intended to help guide decision makers as they plan future efforts to scale-up action against malnutrition in Nigeria and develop nutrition financing plans that bring to bear resources from the health, social protection, education, and agriculture sectors.

    Scaling Up Nutrition : What Will it Cost?

    Undernutrition imposes a staggering cost worldwide, both in human and economic terms. It is responsible for the deaths of more than 3.5 million children each year (more than one-third of all deaths among children under five) and the loss of billions of dollars in forgone productivity and avoidable health care spending. Individuals lose more than 10 percent of lifetime earnings, and many countries lose at least 2-3 percent of their gross domestic product to undernutrition. The current economic crisis and its potential impact on the poor make investing in child nutrition more urgent than ever to protect and strengthen human capital in the most vulnerable developing countries. This report offers suggestions on how to raise these resources. It is an investment we must make. It will yield high returns in the form of thriving children, healthier families, and more productive workers. This investment is essential to make progress on the nutrition and child mortality Millennium Development Goals (MDGs) and to protect critical human capital in developing economies. The human and financial costs of further neglect will be high. This call for greater investment in nutrition comes at a time when global efforts to strengthen health systems provide a unique opportunity to scale up integrated packages of health and nutrition interventions, with common delivery platforms, and lower costs. The report has benefited from the expertise of many international agencies, nongovernmental organizations, and research institutions. The cooperation of so many practitioners is evidence of a growing recognition of the need to invest in nutrition interventions, and a growing consensus about how to deliver effective programs.

    Classroom Assessment to Support Foundational Literacy

    This document focuses primarily on how classroom assessment activities can measure students’ literacy skills as they progress along a learning trajectory towards reading fluently and with comprehension by the end of primary school grades. The document addresses considerations regarding the design and implementation of early grade reading classroom assessment, provides examples of assessment activities from a variety of countries and contexts, and discusses the importance of incorporating classroom assessment practices into teacher training and professional development opportunities for teachers. The structure of the document is as follows. The first section presents definitions and addresses basic questions on classroom assessment. Section 2 covers the intersection between assessment and early grade reading by discussing how learning assessment can measure early grade reading skills following the reading learning trajectory. Section 3 compares some of the most common early grade literacy assessment tools with respect to the early grade reading skills and developmental phases. Section 4 of the document addresses teacher training considerations in developing, scoring, and using early grade reading assessment. Additional issues in assessing reading skills in the classroom and using assessment results to improve teaching and learning are reviewed in section 5. Throughout the document, country cases are presented to demonstrate how assessment activities can be implemented in the classroom in different contexts.

    This year’s Word Development Report (WDR), the twenty-eighth, looks at the role of equity in the development process. It defines equity in terms of two basic principles. The first is equal opportunities: that a person’s chances in life should be determined by his or her talents and efforts, rather than by pre-determined circumstances such as race, gender, social or family background. The second principle is the avoidance of extreme deprivation in outcomes, particularly in health, education and consumption levels. This principle thus includes the objective of poverty reduction. The report’s main message is that, in the long run, the pursuit of equity and the pursuit of economic prosperity are complementary. In addition to detailed chapters exploring these and related issues, the Report contains selected data from the World Development Indicators 2005‹an appendix of economic and social data for over 200 countries. This Report offers practical insights for policymakers, executives, scholars, and all those with an interest in economic development.

    The economy continues to contract, albeit at a somewhat slower pace. Public finances improved in 2021, but only because spending collapsed faster than revenue generation. Testament to the continued atrophy of Lebanon’s economy, the Lebanese Pound continues to depreciate sharply. The sharp deterioration in the currency continues to drive surging inflation, in triple digits since July 2020, impacting the poor and vulnerable the most. An unprecedented institutional vacuum will likely further delay any agreement on crisis resolution and much needed reforms; this includes prior actions as part of the April 2022 International Monetary Fund (IMF) staff-level agreement (SLA). Divergent views among key stakeholders on how to distribute the financial losses remains the main bottleneck for reaching an agreement on a comprehensive reform agenda. Lebanon needs to urgently adopt a domestic, equitable, and comprehensive solution that is predicated on: (i) addressing upfront the balance sheet impairments, (ii) restoring liquidity, and (iii) adhering to sound global practices of bail-in solutions based on a hierarchy of creditors (starting with banks’ shareholders) that protects small depositors.

    Argentina Country Climate and Development Report

    The Argentina Country Climate and Development Report (CCDR) explores opportunities and identifies trade-offs for aligning Argentina’s growth and poverty reduction policies with its commitments on, and its ability to withstand, climate change. It assesses how the country can: reduce its vulnerability to climate shocks through targeted public and private investments and adequation of social protection. The report also shows how Argentina can seize the benefits of a global decarbonization path to sustain a more robust economic growth through further development of Argentina’s potential for renewable energy, energy efficiency actions, the lithium value chain, as well as climate-smart agriculture (and land use) options. Given Argentina’s context, this CCDR focuses on win-win policies and investments, which have large co-benefits or can contribute to raising the country’s growth while helping to adapt the economy, also considering how human capital actions can accompany a just transition.
