Information and Service Delivery: Case Studies from Kenya and Ethiopia

Abstract
This monograph probes the role of information in service delivery by focusing on key sectors in Kenya and Ethiopia. Findings from comparative studies done in 2005-06 in the health, education, and water and sanitation sectors plus public and private partnerships (PPPs) illustrate the significance of information access to delivery of quality services. Linking information access to service delivery is instructive to deepen institutional reforms around transparency and governance. This volume contends that three constraints-governance, trust, and technology deficits-impede information flows for service delivery in Ethiopia and Kenya. Although these constraints are formidable, the cases reveal that reforms in the governance arena have began to make a difference in the domain of service delivery. Using local researchers for this study has contributed to the World Bank's vision and mission of strengthening knowledge production by local institutions, particularly in Africa, where the low rate of the input of indigenous voices in the development debates is a growing concern. Augmenting the corpus of knowledge about African issues by Africans also fits into the objective of capacity building: providing local experts the opportunities to research and highlight experiences on the ground. As more countries embark on public sector reforms that deepen transparent information mechanisms and better service delivery, the need for more of these studies will increase. What the authors of this study have articulated is a rich research agenda that ties information access and service provision-research that should help advance policy dialogue with actors involved in public sector and governance reforms.

    Realizing Rights Through Social Guarantees : An Analysis of New Approaches to Social Policy in Latin America and South Africa

    This paper examines the experiences in four Latin American countries and South Africa in the application of a human rights approach to social policy. The paper applies an analysis based on a social guarantees framework -- an innovative approach to integrate a rights-based perspective into social policy. The social guarantees approach moves beyond a purely normative framework to give concrete meaning to economic, social, and cultural rights and hence to allow for their operationalization into policies and programs. This document suggests that a social guarantee approach can be used to strengthen the delivery and monitoring of social programs. Firstly, it implies an institutional design that emphasizes synergy and coordination among agencies and providers to help social programs achieve their full potential. Secondly, this approach contributes to reducing gaps in opportunity among citizens by promoting universal access to, and a basic quality standard for, essential services. Thirdly, the social guarantee approach contributes to strengthening democratic governance, as it requires the achievement of a non-discriminatory agreement among all members of society as to the level of basic entitlements of each individual or collective. In other words, social guarantees are safeguards that society provides to all its members, ensuring their access to essential opportunities and well-being.

    Increasing Social Inclusion through Social Guarantees : A Policy Note

    This policy note seeks to contribute to the consideration of social guarantees as tools for the design and/or monitoring of social policy and service delivery. It proposes a range of options to improve the delivery of and access to social services that can further social inclusion and democratic governance. The application of a social guarantee framework to social policy could significantly enhance the effectiveness of policy implementation in Latin America and the Caribbean, and can have a direct and positive impact on poor and vulnerable groups. This policy note is not intended as a blueprint for developing a social guarantee approach to social policy design and service delivery. Rather, it discusses the principles that could guide such undertaking, and it provides examples of how different countries have developed and implemented social programs using this approach. Social policy plays a critical role in promoting inclusion and strengthening democratic governance. As such, it contributes to the realization of normative human rights commitments. This document suggests that a social guarantee approach can be used to strengthen the delivery and monitoring of social programs. It highlights the importance of understanding and building upon the existing socio-political context of each country. This policy notes suggests that a system of social guarantees provides a way of addressing these concerns by enabling the state to provide: a) clarity of minimum standards, which can be revised on an ongoing basis; b) a framework for equity between those who receive services from different providers (public, private, voluntary); c) a framework for redress if minimum standards are not met; and d) a process for citizen participation in public and private provision.

    Islamic Republic of Iran - Health Sector Review : Volume 2. Background Sections

    The Government of the Islamic Republic of Iran has stated its commitment to improving the health and nutritional status of the population as articulated in the Interim Country Assistance Strategy (CAS), which lists as priorities addressing nutritional deficiencies, expanding coverage of basic health services, and improving efficiency of the health system and the quality of service at all levels. This health sector review is consistent with this in that it provides the platform and evidence base for such reform. It is also in-line with part three of the current Fourth Five-Year Plan, which includes health development, human security and social justice priority, the reduction of illness from malnutrition, increasing public health service coverage, and increasing access to quality health services while reducing the financial burden on the families. The objective of this study is to provide a comprehensive review and diagnostic of the performance of the health sector in the Islamic Republic of Iran. The government is quite advanced in the type of analysis it already undertakes on various aspects of the sector (e.g., burden of diseases, national health accounts, and utilization analysis). However, a consolidation of this information and analysis that encompasses several major elements of the health sector has not been done recently. The health sector review, through a synthesis of available data and other information on the health sector attempts to: (i) assess the strengths, challenges, and opportunities facing the current health system; (ii) offer analytic assessments of the health policies and plans; and (iii) provide a framework for developing strategic options as well as short- and medium-term recommendations and action plans to achieve the goals identified in the country's Fourth Five-Year Plan. The review is also intended to provide a platform for discussions on possible areas of collaboration between the Government of the Islamic Republic of Iran and the World Bank on the health sector.

    The Health Extension Program in Ethiopia

    Ethiopia has made substantial progress in improving health outcomes during the last decade and is on track to achieve some of the health Millennium Development Goals. Innovative strategies to improve household behaviors and coverage of basic health care services contributed to Ethiopia's achievements, and the Health Extension Program (HEP) remains the core of such innovations and provides a model for countries struggling to improve health outcomes in a resource-constrained setting. The program rests on an accelerated expansion of basic health infrastructure and local human resources with required skills to scale-up delivery of high-impact interventions focusing on improving the supply of and enhancing demand for a well-defined package of essential promotive, preventive, and curative health services. The objectives of the case study are to provide a detailed description of (a) the context for the introduction of the program; (b) the scope of the service package delivered under the program; and (c) the institutional arrangements and the links with the rest of the health system. The case study also summarizes and discusses the evidence of the program's achievements and the challenges to achieving universal primary health care coverage. The study also discusses the importance of political will and commitment in introducing such large-scale innovations in improving service delivery and mobilizing the community in a resource-constrained setting.

    In the past decade, Latin America and the Caribbean has achieved impressive social and economic successes. For the first time in history, more people are in the middle class than in poverty. Inequality, although still high, declined markedly. Growth, jobs and effective social programs have transformed the lives of millions. In a striking departure from the crisis-prone Latin America of the past, the region has shown it is better prepared to weather the brunt of the global economic slowdown. Now, the region faces the challenge of maintaining and expanding its hard won gains in an adverse context of low growth. This is caused in part by a decrease in commodity prices and reduced economic activity in major commercial partners such as China. In such a scenario, achieving development results - and learning from them - becomes more important. This publication showcases stories about people and how their lives have been improved through better health and education, youth employment, disaster recovery and preparedness, infrastructure, and more.

    Early Child Development, From Measurement to Action : A priority for Growth and Equity

    The World Bank recently hosted a symposium on the priority of early child development (ECD) for economic growth and equity. The participants urged application of population-based tools and measures to assess the outcomes of children's early years and children's readiness for school. This study is derived from the symposium and is a valuable resource for policy makers, economists, donors, and investors, as well as researchers and practitioners in early child development. It summarizes the current neuroscience on early child development and major longitudinal studies, the rationale and urgency for greater investment, and countries' innovative funding strategies. The report consists of 15 chapters authored by ECD experts and leaders in the field. The chapters are grouped into five main parts relating to the: business imperative and societal benefits of ECD investments; lessons from evaluation of longitudinal ECD interventions; countries' experiences in monitoring ECD interventions; innovative approaches to countries' financing of ECD initiatives; and next steps on the ECD agenda for the next 5 years. A theme highlighted at the symposium and enlarged upon here is the urgent need for evidence- and population-based instruments and measures to monitor, evaluate, and compare ECD interventions over time and across settings.

    Making Monitoring and Evaluation Systems Work : A Capacity Development Toolkit

    There are constant and growing pressures on governments and organizations around the world to be more responsive to demands from internal and external stakeholders for good governance, accountability and transparency, greater development effectiveness and delivery of tangible results. Governments, parliaments, citizens, the private sector, non-governmental organizations (NGOs), civil society, international organizations, and donors are all among stakeholders interested in better performance. As demands for greater accountability and results have grown, there is an accompanying need for useful and useable results-based monitoring and evaluation systems to support the management of policies, programs, and projects. Governments and other organizations have many different kinds of tracking systems as part of their management toolkits: good human resource systems, financial systems, and accountability systems. They also need good feedback systems. A results-based monitoring and evaluation (M&E) system is essentially such a feedback system; it is a management tool to measure and evaluate outcomes, providing information for governance and decision making. Many management systems have been missing a feedback component to enable them to track the consequences of actions. Building an M&E system gives decision-makers an additional management tool by providing feedback on performance as a basis for future improvement.

    Changing the Face of the Waters : The Promise and Challenge of Sustainable Aquaculture

    This study provides strategic orientations and recommendations for Bank client countries and suggests approaches for the Bank's role in a rapidly changing industry with high economic potential. It identifies priorities and options for policy adjustments, catalytic investments, and entry points for the Bank and other investors to foster environmentally friendly, wealth-creating, and sustainable aquaculture. The objectives of the study are to inform and provide guidance on sustainable aquaculture to decision makers in the international development community and in client countries of international finance institutions. The study focuses on several critical issues and challenges: 1) Harnessing the contribution of aquaculture to economic development, including poverty alleviation and wealth creation, to employment and to food security and trade, particularly for least developed countries (LDCs); 2) Building environmentally sustainable aquaculture, including the role of aquaculture in the broader suite of environmental management measures; 3) Creating the enabling conditions for sustainable aquaculture, including the governance, policy, and regulatory frameworks, and identifying the roles of the public and private sectors; and 4) Developing and transferring human and institutional capacity in governance, technologies, and business models with special reference to the application of lessons from Asia to Sub-Saharan Africa and Latin America.

    Climate Change and Migration : Evidence from the Middle East and North Africa

    Climate change is a major source of concern in the Middle East and North Africa (MENA) region, and migration is often understood as one of several strategies used by households to respond to changes in climate and environmental conditions, including extreme weather events. This study focuses on the link between climate change and migration. Most micro-level studies measure climate change either by the incidences of extreme weather events or by variation in temperature or rainfall. A few studies have found that formal and informal institutions as well as policies also affect migration. Institutions that make government more responsive to households (for example through public spending) discourage both international and domestic migration in the aftermath of extreme weather events. Migration is often an option of last resort after vulnerable rural populations attempting to cope with new and challenging circumstances have exhausted other options such as eating less, selling assets, or removing children from school. This study is based in large part on new data collected in 2011 in Algeria, Egypt, Morocco, Syria, and the Republic of Yemen. The surveys were administered by in-country partners to a randomly selected set of 800 households per country. It is also important to emphasize that neither the household survey results nor the findings from the qualitative focus groups are meant to be representative of the five countries in which the work was carried, since only a few areas were surveyed in each country. This report is organized as follows: section one gives synthesis. Section two discusses household perceptions about climate change and extreme weather events. Section three focuses on migration as a coping mechanisms and income diversification strategy. Section four examines other coping and adaptation strategies. Section five discusses perceptions about government and community programs.
