Abstract
This report contributes to the literature on the economic implications of non-communicable disease (NCDs) in developing countries by focusing on the case of India. First, the authors examine available evidence on the economic impact of NCDs in India. Second, the authors present new estimates of impact of NCDs, both on household economic well being as well as on aggregate economic outcomes in India. India is a worthwhile case to study for many reasons, beginning with the large numbers involved. In the year 2004, an estimated 8.1 million Indians died from all causes. Based on WHO data on the distribution of deaths by cause, estimated deaths from non-communicable conditions (including injuries) amounted roughly to 4.8 million, or about 59.4 percent of all deaths in the year. Moreover, about 24 percent of these deaths from non-communicable conditions occurred in the 35-64 age groups as per data from medically certified deaths from hospitals located in urban areas of India. Available data also indicate that 34 percent of all injury-related deaths occurred among individuals in the 35-64 year age group. India's population is also ageing over time, an issue of concern given the higher incidence of NCDs in older age groups. All of these developments are occurring in a setting where health expenditures are growing rapidly led by an unregulated private sector and where health insurance and pension coverage are still limited. These financial concerns are further exacerbated by the emerging evidence that the India's poor are at heightened risk of acquiring NCDs owing to high rates of smoking and tobacco use, occupational risks, and residential living conditions.

    The Growing Burden of Non-Communicable Diseases in the Eastern Caribbean

    The purpose of this report is to examine and propose options for policy design and implementation, based on a situational analysis for six Eastern Caribbean countries and a review of the lessons of other nations in prevention and control. A committed partner of the Organization of Eastern Caribbean States (OECS) countries, the World Bank is responding to their needs to address the increasing Non-Communicable Diseases (NCDs) burden by providing this report. This report analyzes the situation, reviews lessons learnt in other countries in the prevention and control of major NCDs, and proposes policy options to address the challenges. Specifically, this report aims to provide policy makers with an overview of the NCDs and key risk factors in the region and make inter-country comparisons; assess the economic impact of key NCDs and risk factors where data are available; outline policy options and cost effective interventions for controlling major NCDs that are common to the OECS countries; and provide input for developing a regional health strategy. In the belief that powerful information can drive strategic action, the report aims to: raise awareness of the reality and consequences of failure to take strong action; and identify priority actions that are realistic and tangible for policy makers in developing strategies and allocating resources.

    Non-Communicable Diseases in Jamaica

    Jamaica is a Caribbean country that has initiated comprehensive programs to address Non-Communicable Diseases (NCDs). The government created the National Health Fund (NHF) to reduce the cost of treatment of NCDs and finance some prevention programs. The main objective of this study is to learn from Jamaica's experience in tackling major NCDs and related risk factors, to provide policy options for Jamaica to improve its NCD programs and to share with other countries the lessons learned from its experience. The study attempts to answer three questions: a) whether the NHF and its drug subsidy program have reduced out-of-pocket spending on NCDs; b) whether access to treatment of NCDs has improved; and c) what the economic burden on NCD patients and their families is. The report presents an overall picture of the epidemiological and demographic transitions in Jamaica, its current burden of NCDs, and the change in the trend of NCDs in the past decade, using publicly available data, particularly data from the Jamaica living condition household surveys. It assesses the risk factors and analyzes Jamaica's response to NCDs with emphasis on the impact of the NHF on people's lives. Estimates of the economic burden of NCDs are provided and policy options to improve Jamaica's NCD programs are suggested. This study focuses on Jamaica's experience in addressing major NCDs and their related risk factors with the objective of learning from Jamaica and providing policy options to Jamaica to improve its programs.

    The Economic Costs of Noncommunicable Diseases in the Pacific Islands

    There are three main messages running throughout this report. First, Non Communicable Diseases (NCDs) can impose large health, financial and economic costs on countries. This is particularly important in the Pacific where Government already finances, and provides, the bulk of health services. Second, risk factors in the Pacific are feeding a pipeline of potentially expensive to treat NCDs, including diabetes and heart disease, but governments are already fiscally constrained in how much more they can provide to the health system. Third, from public health and public finance perspective, many of the NCDs are avoidable, or their health and financial costs can at least be postponed, through good primary and secondary prevention. This will require a more coherent approach to health system financing, and health system operations more generally.

    There is increasing recognition that non-communicable diseases (NCDs) are an important international and development issue globally, undermining health gains and imposing financial and economic costs on governments and households. NCDs are an important health challenge in the Pacific. First, (NCDs) can impose large but often preventable health, financial, and economic costs on countries. This is particularly important in the Pacific, where government already finances and provides the bulk of health services. Second, risk factors in the Pacific are feeding a pipeline of potentially expensive-to-treat NCDs, including diabetes and heart disease, but governments are already fiscally constrained in how much more they can provide to the health system. Third, from a public health and public finance perspective, many of the NCDs are avoidable or their health and financial costs can at least be postponed through good primary and secondary prevention. This will require a more coherent approach to health system financing and to health system operations, more generally. Improving both allocative efficiency ("doing the right things") and technical efficiency ("doing things right") are critical strategies to improve health outcomes in a financially sustainable way in the resource-constrained Pacific.

    Toward a Healthy and Harmonious Life in China

    China's 12th five-year plan (2011-2015) aims to promote inclusive, equitable growth and development by placing an increased emphasis on human development. Good health is an important component of human development, not only because it makes people's lives better, but also because having a healthy and long life enhances their ability to learn, acquire skills, and contributes to society. Indeed, good health is a fundamental right of every human being. Good health among a population can also enhance economic performance by improving labor productivity and reducing economic losses that arise from illnesses. The findings and recommendations can inform and promote a broad dialogue toward the development of a multisectoral response to effectively address the growing burden of Non Communicable Diseases (NCDs), including a better alignment of the health system with the population's health needs. The report also advocates implementing 'health in all' policies and actions for a multisectoral response to NCDs in China to help achieve the ultimate goal of 'harmonious' development and growth.

    Global growth is projected to slow significantly in the second half of this year, with weakness continuing in 2024. Inflation pressures persist, and tight monetary policy is expected to weigh substantially on activity. The possibility of more widespread bank turmoil and tighter monetary policy could result in even weaker global growth. Rising borrowing costs in advanced economies could lead to financial dislocations in the more vulnerable emerging market and developing economies (EMDEs). In low-income countries, in particular, fiscal positions are increasingly precarious. Comprehensive policy action is needed at the global and national levels to foster macroeconomic and financial stability. Among many EMDEs, and especially in low-income countries, bolstering fiscal sustainability will require generating higher revenues, making spending more efficient, and improving debt management practices. Continued international cooperation is also necessary to tackle climate change, support populations affected by crises and hunger, and provide debt relief where needed. In the longer term, reversing a projected decline in EMDE potential growth will require reforms to bolster physical and human capital and labor-supply growth.

    All African countries need better and more jobs for their growing populations. "Digital Africa: Technological Transformation for Jobs" shows that broader use of productivity-enhancing, digital technologies by enterprises and households is imperative to generate such jobs, including for lower-skilled people. At the same time, it can support not only countries’ short-term objective of postpandemic economic recovery but also their vision of economic transformation with more inclusive growth. These outcomes are not automatic, however. Mobile internet availability has increased throughout the continent in recent years, but Africa’s uptake gap is the highest in the world. Areas with at least 3G mobile internet service now cover 84 percent of Africa’s population, but only 22 percent uses such services. And the average African business lags in the use of smartphones and computers as well as more sophisticated digital technologies that catalyze further productivity gains. Two issues explain the usage gap: affordability of these new technologies and willingness to use them. For the 40 percent of Africans below the extreme poverty line, mobile data plans alone would cost one-third of their incomes—in addition to the price of access devices, apps, and electricity. Data plans for small- and medium-size businesses are also more expensive than in other regions. Moreover, shortcomings in the quality of internet services—and in the supply of attractive, skills-appropriate apps that promote entrepreneurship and raise earnings—dampen people’s willingness to use them. For those countries already using these technologies, the development payoffs are significant. New empirical studies for this report add to the rapidly growing evidence that mobile internet availability directly raises enterprise productivity, increases jobs, and reduces poverty throughout Africa. To realize these and other benefits more widely, Africa’s countries must implement complementary and mutually reinforcing policies to strengthen both consumers’ ability to pay and willingness to use digital technologies. These interventions must prioritize productive use to generate large numbers of inclusive jobs in a region poised to benefit from a massive, youthful workforce—one projected to become the world’s largest by the end of this century.

    Europe and Central Asia Economic Update, Fall 2024: Better Education for Stronger Growth

    Economic growth in Europe and Central Asia (ECA) is likely to moderate from 3.5 percent in 2023 to 3.3 percent this year. This is significantly weaker than the 4.1 percent average growth in 2000-19. Growth this year is driven by expansionary fiscal policies and strong private consumption. External demand is less favorable because of weak economic expansion in major trading partners, like the European Union. Growth is likely to slow further in 2025, mostly because of the easing of expansion in the Russian Federation and Turkiye. This Europe and Central Asia Economic Update calls for a major overhaul of education systems across the region, particularly higher education, to unleash the talent needed to reinvigorate growth and boost convergence with high-income countries. Universities in the region suffer from poor management, outdated curricula, and inadequate funding and infrastructure. A mismatch between graduates' skills and the skills employers are seeking leads to wasted potential and contributes to the region's brain drain. Reversing the decline in the quality of education will require prioritizing improvements in teacher training, updated curricula, and investment in educational infrastructure. In higher education, reforms are needed to consolidate university systems, integrate them with research centers, and provide reskilling opportunities for adult workers.

    Global growth is expected to hold steady at 2.7 percent in 2025-26. However, the global economy appears to be settling at a low growth rate that will be insufficient to foster sustained economic development—with the possibility of further headwinds from heightened policy uncertainty and adverse trade policy shifts, geopolitical tensions, persistent inflation, and climate-related natural disasters. Against this backdrop, emerging market and developing economies are set to enter the second quarter of the twenty-first century with per capita incomes on a trajectory that implies substantially slower catch-up toward advanced-economy living standards than they previously experienced. Without course corrections, most low-income countries are unlikely to graduate to middle-income status by the middle of the century. Policy action at both global and national levels is needed to foster a more favorable external environment, enhance macroeconomic stability, reduce structural constraints, address the effects of climate change, and thus accelerate long-term growth and development.

Global growth is expected to slow further this year, reflecting the lagged and ongoing effects of tight monetary policy to rein in inflation, restrictive credit conditions, and anemic global trade and investment. Downside risks include an escalation of the recent conflict in the Middle East, financial stress, persistent inflation, weaker-than-expected activity in China, trade fragmentation, and climate-related disasters. Against this backdrop, policy makers face enormous challenges. In emerging market and developing economies (EMDEs), commodity exporters face the enduring challenges posed by fiscal policy procyclicality and volatility, which highlight the need for robust fiscal frameworks. Across EMDEs, previous episodes of investment growth acceleration underscore the critical importance of macroeconomic and structural policies and an enabling institutional environment in bolstering investment and long-term growth. At the global level, cooperation needs to be strengthened to provide debt relief, facilitate trade integration, tackle climate change, and alleviate food insecurity.
