Building the Foundation for Healthy Societies: Influencing Multisectoral Action for Health, Phase One, Volume 1. Multisectoral Opportunities and Constraints Assessment Tool

Abstract
The KP aims to support the Bank in adopting a significantly more systematic approach to multisectoral action in countries to maximize investment in other sectors for health results, in line with its comparative advantage. It will help to meet the Bank s twin goals and add value to its work in countries, by providing task teams with data on the burden of disease and risks and their upstream determinants, which multisectoral action could address, toward the goal of reducing poverty. It will assist in demonstrating the links between upstream determinants, such as poverty and inequality, and health outcomes. KP will identify proven and cost-effective multisectoral solutions from international best practice that have the potential for transferability to local contexts. It will then help understand the drivers, opportunities and constraints of work across practices and sectors on a common agenda to meet the Bank s goals. Finally, contributing to the effective delivery of existing Bank work across sectors within countries, and provide foresight and priorities for future action.

    Effective Responses to Non-communicable Diseases : Embracing Action Beyond the Health Sector

    Effective responses to non-communicable disease: Embracing action beyond the health sector focuses on solutions, indicating opportunities for the prevention and control of non-communicable diseases (NCDs) and the kinds of actions that will achieve it. NCDs exact a heavy toll on individuals and society. They cause disease, disability, and death, and reduce productivity which is vital for development. They also impose hefty costs on health services, particularly since NCDs frequently lead to ongoing disability and need for long-term care. Prevention that results in healthy aging and the reduction of morbidity is far more cost-effective and financially sustainable than treatment alone. A small number of proven prevention measures could stem the rise of the NCD epidemic. Data indicate the potential of affordable, potent, prevention tools focused on five key areas: tobacco, alcohol, diet, exercise and nutrition. Together, programs targeting these areas could reduce the burden of NCDs by more than half, while costing only a tiny fraction of current health spending. Since successful prevention involves multiple sectors and actors, countries will need to adopt a framework that clearly sets out the different levels of accountability of each role player. Such a framework will require strong national buy-in, but there are tools, examples, and support agencies available to facilitate this. Middle- and low-income countries can seize the opportunity and act now to tackle NCDs and the underlying risk factors of these diseases. If they do so, these countries will greatly increase their prospects of reaping the full benefits of their demographic dividend, and of ensuring sustained economic development, greater equality, and a better quality of life for their people in the years ahead.

    Tackling NCDIs in Cambodia : An Opportunity for Inter - and Itra-Sectoral Synergies

    Non-communicable diseases (NCDs) are a set of chronic diseases, including cardiovascular disease, cancer, chronic respiratory disease, and diabetes that are on the rise in Cambodia. Meanwhile, injuries include those due to traffic accidents, which are becoming a major source of mortality and morbidity in Cambodia. NCDs are increasing in Cambodia as a result of smoking, unhealthy diet, harmful use of alcohol, and physical inactivity, all resulting in a sharp increase in the rates of obesity and high blood pressure. NCDs are affecting Cambodians in their productive years. Over half men and over a third of women dying from NCDs are younger than 60 years. The high cost of long-term health care and medicines, along with loss of income, pushes many Cambodian families deeper into poverty. It also overburdens the country's health system, constraining efforts to improve maternal and child health and tackle infectious diseases. Because of the nature of NCDs, which are rooted in multiple causes, strengthening the health system alone will not reduce the burden of NCDs. Many of the actions needed to prevent and control these diseases require the participation of numerous ministries and government agencies, as well as the private sector. For Cambodians to continue enjoying the fruits of economic growth, a strong commitment by the government, the people, and the international community is becoming increasing important. Improved coordination among all ministry of health (MOH) task forces will maximize financial and human resources and help implement priority NCDI-related interventions as part of, and not separate from, other ongoing programs. The application of the measures will benefit from clear procedures for joint planning, programming, budgeting, training, reporting, and monitoring and evaluation (M and E) of all programs.

    Transport for Health : The Global Burden of Disease from Motorized Road Transport

    This report summarizes the findings of a long and meticulous journey of data gathering and analysis to quantify the health losses from road deaths and injuries worldwide, as part of the path-finding Global Burden of Disease (GBD) study. It is important, first, to acknowledge the profound contribution made by the lead authors and global team of injury prevention professionals to estimate the disease burden of road trauma, before absorbing their findings and recommendations. Without their dedication and tenacity, the way forward would be less certain. The first GBD study, published nearly two decades ago, signaled an emerging road safety crisis in developing regions of the world. It triggered a remarkable program of global advocacy that culminated in the United Nations decade of action for road safety and global plan to bring road safety outcomes under control in these regions by 2020. However, limited investment has been mobilized so far to implement the UN initiative. The second GBD studies, and related analyses presented in this report, confirm the importance of road safety as a global development priority and the urgency with which it must be addressed. The report's findings highlight the growth in road deaths and injuries globally, and their substantial impacts on maternal and child health, despite sustained reductions over the last three to four decades in high-income countries. Combined with the deaths arising from vehicle pollution, the road transport death toll exceeds that of, for example, HIV/AIDS, tuberculosis, malaria, or diabetes. This statistic further reinforces the call for global action. Without these GBD estimates we would not have a clear picture of the true situation because official country data in the developing world vastly understate the scale of road transport health losses.

    The Health Benefits of Transport Projects : A Review of the World Bank Transport Sector Lending Portfolio

    This paper reviews the contribution of the World Bank's transport lending portfolio to health outcomes, as background for the Independent Evaluation Group's (IEG) evaluation of the Bank's support for health, nutrition and population (HNP). Over the past decade (FY97-06), the World Bank committed nearly $28 billion to 229 new transport projects managed by the Transport Sector Board (TSB). Specifically, the paper reviews the extent to which these projects: cite potential health benefits or risks in design documents; include specific objectives with respect to improving health outcomes or mitigating health risks; propose environmental improvements that are likely to provide health benefits; target transport services, and both health and behavioral outcomes to the poor; and plans to collect evidence on changes in health outcomes as a result of transport interventions. For completed projects, it assesses the extent to which expected health benefits or objectives have been achieved. This review of health benefits in the transport lending portfolio over the past decade shows that in the majority of the cases the focus has been on improvements in road safety, as measured by a reduction in the fatality or injury rates. Among the 55 projects with health related objectives, 82 percent had objectives to improve road safety, while only 7 percent had objectives to improve safety of other modes of transport and only 2 percent each had HIV/AIDS prevention or air quality improvement objectives.

    World Bank Group President David Malpass spoke about the world facing major challenges, including COVID, climate change, rising poverty and inequality and growing fragility and violence in many countries. He highlighted vaccines, working closely with Gavi, WHO, and UNICEF, the World Bank has conducted over one hundred capacity assessments, many even more before vaccines were available. The World Bank Group worked to achieve a debt service suspension initiative and increased transparency in debt contracts at developing countries. The World Bank Group is finalizing a new climate change action plan, which includes a big step up in financing, building on their record climate financing over the past two years. He noted big challenges to bring all together to achieve GRID: green, resilient, and inclusive development. Janet Yellen, U.S. Secretary of the Treasury, mentioned focusing on vulnerable people during the pandemic. Kristalina Georgieva, Managing Director of the International Monetary Fund, focused on giving everyone a fair shot during a sustainable recovery. All three commented on the importance of tackling climate change.

    Remarks at the United Nations Biodiversity Conference

    World Bank Group President David Malpass discussed biodiversity and climate change being closely interlinked, with terrestrial and marine ecosystems serving as critically important carbon sinks. At the same time climate change acts as a direct driver of biodiversity and ecosystem services loss. The World Bank has financed biodiversity conservation around the world, including over 116 million hectares of Marine and Coastal Protected Areas, 10 million hectares of Terrestrial Protected Areas, and over 300 protected habitats, biological buffer zones and reserves. The COVID pandemic, biodiversity loss, climate change are all reminders of how connected we are. The recovery from this pandemic is an opportunity to put in place more effective policies, institutions, and resources to address biodiversity loss.

    Media and Messages for Nutrition and Health

    The Lao People’s Democratic Republic (Lao PDR) has experienced rapid and significant economic growth over the past decade. However, poor nutritional outcomes remain a concern. Rates of childhood undernutrition are particularly high in remote, rural, and upland areas. Media have the potential to play an important role in shaping health and nutrition–related behaviors and practices as well as in promoting sociocultural and economic development that might contribute to improved nutritional outcomes. This report presents the results of a media audit (MA) that was conducted to inform the development and production of mass media advocacy and communication strategies and materials with a focus on maternal and child health and nutrition that would reach the most people from the poorest communities in northern Lao PDR. Making more people aware of useful information, essential services and products and influencing them to use these effectively is the ultimate goal of mass media campaigns, and the MA measures the potential effectiveness of media efforts to reach this goal. The effectiveness of communication channels to deliver health and nutrition messages to target beneficiaries to ensure maximum reach and uptake can be viewed in terms of preferences, satisfaction, and trust. Overall, the four most accessed media channels for receiving information among communities in the study areas were village announcements, mobile phones, television, and out-of-home (OOH) media. Of the accessed media channels, the top three most preferred channels were village announcements (40 percent), television (26 percent), and mobile phones (19 percent). In terms of trust, village announcements were the most trusted source of information (64 percent), followed by mobile phones (14 percent) and television (11 percent). Hence of all the media channels, village announcements are the most preferred, have the most satisfied users, and are the most trusted source of information in study communities from four provinces in Lao PDR with some of the highest burden of childhood undernutrition.

    Climate Change and Migration : Evidence from the Middle East and North Africa

    Climate change is a major source of concern in the Middle East and North Africa (MENA) region, and migration is often understood as one of several strategies used by households to respond to changes in climate and environmental conditions, including extreme weather events. This study focuses on the link between climate change and migration. Most micro-level studies measure climate change either by the incidences of extreme weather events or by variation in temperature or rainfall. A few studies have found that formal and informal institutions as well as policies also affect migration. Institutions that make government more responsive to households (for example through public spending) discourage both international and domestic migration in the aftermath of extreme weather events. Migration is often an option of last resort after vulnerable rural populations attempting to cope with new and challenging circumstances have exhausted other options such as eating less, selling assets, or removing children from school. This study is based in large part on new data collected in 2011 in Algeria, Egypt, Morocco, Syria, and the Republic of Yemen. The surveys were administered by in-country partners to a randomly selected set of 800 households per country. It is also important to emphasize that neither the household survey results nor the findings from the qualitative focus groups are meant to be representative of the five countries in which the work was carried, since only a few areas were surveyed in each country. This report is organized as follows: section one gives synthesis. Section two discusses household perceptions about climate change and extreme weather events. Section three focuses on migration as a coping mechanisms and income diversification strategy. Section four examines other coping and adaptation strategies. Section five discusses perceptions about government and community programs.
