Abstract
Understanding the economic costs attributable to unhealthy diets is essential for advocating health and agrifood investments in low- and middle-income countries (LMICs) experiencing nutrition transitions. These costs encompass direct and hidden expenses, including absenteeism, presenteeism, and premature mortality. Recent estimates highlight that overweight and obesity contribute 2.2% of global GDP, with significant human costs reflected in millions of deaths associated with noncommunicable diseases (NCDs), predominantly in LMICs. The World Bank projects that by 2035, obesity's hidden costs in LMICs will reach US$7 trillion. The State of Food and Agriculture (SOFA) 2024 report estimates global agrifood systems' hidden costs at over US$10 trillion at 2020 purchasing power parity, with 70% linked to unhealthy diets. These findings underscore the need for reformulating agrifood fiscal policies to promote healthy diets and reduce economic losses.
This report employs a novel methodology to assess the economic costs of child stunting and adult diet-related NCDs attributable to unhealthy diets. Using the Lives Saved Tool (LiST), the study estimates that significant portions of child stunting costs in Ethiopia and the Philippines are due to suboptimal diets. For adult populations, the Global Diet Quality Score (GDQS) was utilized to analyze diet quality and its association with NCD risk factors such as high body mass index (BMI), systolic blood pressure (SBP), and fasting blood glucose (FBG). Results indicate widespread unhealthy diets across all BMI categories, suggesting that estimates based solely on overweight and obesity may underestimate actual costs.
The report concludes that while the methodology provides valuable insights into the economic impacts of unhealthy diets, limitations such as outdated data and the need for robust dietary intake measures in LMICs must be addressed. Further research is necessary to refine these estimates and support policy development aimed at reducing diet-related economic burdens.

Publication Diet Quality, Child Health, and Food Policies in Developing Countries(World Bank Group, Washington, DC, 2014-10)Although the importance of diet quality for improving child health is widely recognized, the roles of environmental factors and the absorption of nutrients for children's physical growth and morbidity have not been adequately integrated into a policy framework. Moreover, nutrient intakes gradually affect child health, so it is helpful to use alternative tools to evaluate short-term interventions versus long-term food policies. This article emphasizes the role of diet quality reflected in the intake of nutrients such as protein, calcium, and iron for children's physical growth. Vitamins A and C are important for reducing morbidity. Children's growth and morbidity affect their cognitive development, which is critical for the future supply of skilled labor and economic growth. Evidence on these issues from countries such as Bangladesh, India, Kenya, the Philippines, and Tanzania is summarized. The supply of nutritious foods is appraised from the viewpoint of improving diet quality. Finally, the roles of educational campaigns and indirect taxes on unhealthy processed foods consumed by the affluent in developing countries are discussed.Publication Situational Analysis Improving Economic Outcomes by Expanding Nutrition Programming in Tajikistan(World Bank, Washington, DC, 2012-02-08)Undernutrition in Tajikistan remains an important public health challenge, albeit a hidden problem. Stunting, iodine deficiency, and maternal and child anemia represent the largest burden of undernutrition in Tajikistan. In 2009, around 29 percent of children in all regions of the country were stunted. Iodine deficiency was observed in 53 percent of children and in 58.6 percent of women. The national prevalence of anemia in children was 28.8 percent; however, rates were as high as 39.8 percent in Ghorno-Badakhshan Autonomous Province and in 32 percent directly ruled districts (DRD). The prevalence of anemia among mothers was 24.2 percent. The long-term effects of these conditions negatively affect the health of adults throughout their life, as well as their potential productivity in the work force and possible economic contribution to the nation. The highest priority interventions will improve infant and young child feeding. Strengthening and scaling up breastfeeding promotion will save lives and help to reduce stunting as would complementary feeding for babies six months and older. Promoting exclusive breastfeeding for infants under six months is the most efficacious intervention to save lives, averting nearly 20 percent of deaths in children under-five. Effective programs need to be designed to make more women aware of the benefits of breastfeeding and sound nutrition. A comprehensive, multi-sector approach is needed to ensure success of these interventions. This report also makes several other recommendations including: supplementing pregnant women with either iron folic acid or multiple micronutrients, maintain twice-annually vitamin A supplementation, implementing a deworming program, support for flour fortification, scale up and maintain zinc for the management of diarrhea, and address the underlying and basic causes of undernutrition through other sectors.Publication The Economic Costs of Noncommunicable Diseases in the Pacific Islands : A Rapid Stocktake of the Situation in Samoa, Tonga, and Vanuatu(World Bank, Washington, DC, 2013-09)There is increasing recognition that non-communicable diseases (NCDs) are an important international and development issue globally, undermining health gains and imposing financial and economic costs on governments and households. NCDs are an important health challenge in the Pacific. First, (NCDs) can impose large but often preventable health, financial, and economic costs on countries. This is particularly important in the Pacific, where government already finances and provides the bulk of health services. Second, risk factors in the Pacific are feeding a pipeline of potentially expensive-to-treat NCDs, including diabetes and heart disease, but governments are already fiscally constrained in how much more they can provide to the health system. Third, from a public health and public finance perspective, many of the NCDs are avoidable or their health and financial costs can at least be postponed through good primary and secondary prevention. This will require a more coherent approach to health system financing and to health system operations, more generally. Improving both allocative efficiency ("doing the right things") and technical efficiency ("doing things right") are critical strategies to improve health outcomes in a financially sustainable way in the resource-constrained Pacific.Publication Nutritional Failure in Ecuador : Causes, Consequences, and Solutions(Washington, DC: World Bank, 2007)This study supports the development of a more coherent and effective nutrition strategy in Ecuador through an analysis of the main nutrition issues, based on in-depth statistical analysis of a large new household survey dataset (ENDEMAIN 2004) and other data sources, together with a review of qualitative evidence regarding behavioral and program-access obstacles to improved nutritional outcomes. It also reviews the existing programs and policies which aim to improve nutritional outcomes, considered the available evidence on the efficiency, effectiveness, targeting and inter-programmatic coherence of the programs and projects reviewed and suggests an agenda for policy discussions to improve these outcomes.Publication How to Protect and Promote the Nutrition of Mothers and Children(World Bank, Washington, DC, 2012-12-10)The nutrition cluster is a very important entity to coordinate actions during emergencies. It is important that the nutrition cluster know in advance which institutions work where and what kind of inputs are pre-positioned. Risk management plans need to be ready at the local level in advance of emergencies. Although emergencies occur every year in Guatemala, the emergency response often fails to incorporate the management of malnutrition among its priority actions. Community programs, like AIN-C (a community- based child care program - atencion integral a la ninez comunitaria), can be an efficient mechanism to protect young children's nutrition and provide an important channel for the government or other assistance agencies to funnel support to communities in need during a period of crisis or emergency. This is because community workers know the families and those who are most vulnerable; they are willing to be called upon to help their community; and they can provide educational support to families to ensure rapid recovery among young children. Community-based growth promotion programs such as AIN-C can be strengthened and scaled up, and they are a good investment, in the aftermath of an emergency or during a time of economic crisis, to swiftly deliver services to affected families. Community agents can carry important information to the community and can distribute food, nutrition and health supplements such as micronutrient powders and oral rehydration salts for young children, as well as hygiene and water purification products. Community kitchens are an efficient approach to reduce hunger among the poor during times of high economic stress. They provide a social safety net and can have a nutrition effect when carefully planned. Community kitchens adjust to the labor market and general economic conditions; hence targeting of the poor and most in need through community kitchens is self-selective. The kitchens can expand and shrink as participants continually assess the trade-off between unpaid work in exchange for free or inexpensive meals and the pursuit of opportunities in the labor market.

Publication Mapping Impact In Chad(Washington, DC: World Bank, 2025-06-25)In the Sahel, Adaptive Social Protection (ASP) is a set of social protection policies, systems, and programs that promote human capital, productivity, and resilience of the poorest and strengthen their capacity to prepare for, cope with, and adapt to shocks. Through the delivery of regular social safety nets, productive inclusion interventions, and shock-responsive programs, ASP has demonstrated strong positive impacts on various dimensions in the Sahel. For the poorest and most vulnerable, it has resulted in improvements in household welfare and food security, productivity, and resilience. More broadly, it has shown significant positive impacts on the economy, society, and future generations.Publication Indonesia Economic Prospects, June 2025(Washington, DC: World Bank, 2025-06-23)Indonesia’s economy remains resilient amid worsening global conditions. GDP grew at 4.9 percent year-on-year (yoy) in Q1-2025, slightly lower than previous post-pandemic quarters. Domestic demand was impacted by reduced government consumption and lower investment. Budget efficiency measures led to a contraction in public consumption, while investment in the construction and manufacturing sectors dipped due to investors’ concerns over domestic and global policy uncertainty. Meanwhile, declining commodity prices worsened Indonesia’s terms of trade. The supply side showed notable contributions from the agriculture and services sectors. Businesses and households are adjusting to economic uncertainty, but weak consumption of middle-class households has been persistent since the pandemic. The GOI structural reform agenda could accelerate growth further. In response to rising global policy uncertainty, the GOI devised a program of deregulation including reforms to the business environment and licensing, investment liberalization, trade and logistics reforms, and digital services. These reforms complement other reforms currently in play, like those related to financial sector deepening, and accompany the demand stimulus that the GOI is targeting through its priority programs. If implemented, these reforms could gradually expand the economy’s capacity, unlock further FDI, boost investment returns, and ensure productivity gains. The report suggests that this will translate into better job creation and raise GDP growth to 5.3-5.5 percent in 2026-2027. This report identifies the necessary steps to reach the target of providing 3 million housing units each year. In short, to meet the housing target and supercharge current efforts, the government needs to act as both a housing provider and a housing facilitator: instituting housing regulation reforms, accelerating public-funded housing programs, and creating an enabling environment that attracts private investment in Indonesia. Directly, 3.8 billion dollars in annual public investments can create an estimated 2.3 million jobs and mobilize 2.8 billion dollars in private capital. Beyond short-term stimulus, China will need to rely more on household consumption as an engine of growth. Temperatures in the Sahel are rising 1.5 times faster than the global average, leading to extreme temperatures and more erratic and intense rainfall. Climate-related shocks are becoming more frequent and severe, particularly droughts and floods. These crises risk pushing 13.5 million more Sahelians into poverty by 2050 and deepening cycles of fragility and vulnerability, if urgent adaptation measures are not taken.Publication Algeria Economic Update, Spring 2025(Washington, DC: World Bank, 2025-06-20)Algeria’s economic growth remained robust in 2024 but is expected to slow moderately in 2025. Strong investment momentum and robust growth in household consumption, both fueled by government spending, supported manufacturing and services activity, while agricultural production accelerated. However, growth in domestic demand boosted imports, which, combined with lower hydrocarbon production and exports, weighed on growth. Overall, non-hydrocarbon GDP grew at a pace of 4.8 percent, offsetting the 1.4 percent contraction in GDP from hydrocarbons. Real GDP growth is projected at 3.3 percent in 2025, driven by the rebound in growth in the hydrocarbon sectors (+1.6 percent), boosted by the recovery of OPEC production quotas and gas production. Non-hydrocarbon growth is expected to slow (+3.6 percent), driven by the expected consolidation of public spending, which would be more marked for investment. Agricultural production is expected to remain robust despite limited rainfall, offsetting the slowdown in industry and services. The analysis of productivity trends in different sectors offers avenues for reflection to accelerate the structural transformation of the Algerian economy. The public-spending-led growth model resulted in important economic and social achievements in the 2000s, before slowing down in the last decade as the pace of spending growth became unsustainable. In doing so, this growth model has steered employment to low-value-added sectors, including non-commercial services and construction. In addition, a comparative analysis of Algerian productivity suggests a heterogeneous performance, with strong momentum in the agricultural sector contrasting with limited gains in the manufacturing sector. Thus, a growth acceleration could be achieved by increasing productivity gains in the manufacturing and services sectors, on the one hand, and a gradual reallocation of employment to high-value-added sectors on the other, combined with a gradual rebalancing of public spending. Such an economic transformation calls for targeted cross-cutting and sectoral policies to support growth and jobs in the private sector, while equipping workers with the necessary skills.
