Abstract
This health sector profile is a description of the health sector in Bangladesh, including its structure, resources, services, performance and dynamics. This document provides an overview of the sector, based on publicly available reports and data. The document intends to help the Ministry of Health and Family Welfare (MOHFW) and its partners to agree on the way forward and its respective roles in the process. It is neither an evaluation nor a programme review, rather it draws and extracts from these key documents to describe how the system works. Chapters one and two provide an overview on the health status of the people of Bangladesh and the determinants of health. Chapters three to eight summarise relevant aspects of the six health systems building blocks: the overall organisation of the health sector including governance and leadership; health services; human resources; information; financing; and medicines. Chapter nine summarises system reform aspects in the Health, Nutrition and Population Sector Programme (HNPSP) and work leading to the new draft national health policy. The annex provides a list of people consulted. The focus of the health sector profile is to describe how the sector works rather than how it should, or what has not happened in the past. The document offers no recommendations of its own in an attempt to leave open space for dialogue. To best serve this purpose, the profile describes all building blocks rather than only those areas where data and information are available. The expectation is that the profile will be updated periodically to fill existing gaps and reflect changes in the sector over time.

Publication "...and then she died" : Indonesia Maternal Health Assessment(World Bank, 2010-02-01)Maternal health remains one of the top priorities of the Government of Indonesia (GoI) and the Ministry of Health (MoH) because reductions in maternal mortality have been slows. The current Maternal Mortality Ratio (MMR) for Indonesia is 228 but with existing programs and interventions the government does not believe it will be able to achieve its stated millennium development goal of 102 maternal deaths per 100,000 live births by 2015. There are positive trends in an increased use of skilled birth attendants, almost universal access to some level of antenatal care and continued use of family planning but these are not enough to stem the tide of maternal death. Interventions by the skilled birth attendants in many cases are not in line with existing standards and prove to be ineffective in trying to address the emergence of complications. Antenatal care is important, but it is not where gains in decreased mortality will be obtained. The continued use of traditional birth attendants (TBA) and delivering at home are contributory factors to maternal mortality in Indonesia. In collecting data from verbal autopsies in three districts, (a widely used government initiative to determine the causes of maternal death) 63 out of 76 deaths occurred in home births that had been assisted by a TBA. In order to improve the access of the poor and the near poor to health services, the government is implementing the social insurance scheme, Jamkesmas. Approximately 104 million people should qualify for assistance but there are presently only 76.4 million people covered by this social health insurance (SHI) plan. Even those who are covered are not always able to afford care because of external expenses such as transportation charges which must be paid up front.Publication Delivering Better Health Services to Pakistan's Poor(Washington, DC, 2010-04)Pakistan is not on track to achieve most Millennium Development Goals (MDGs) related to health, nutrition and population. Given its current rate of progress, in 2015 Pakistan's infant mortality rate (IMR) will be 65 deaths per 1,000 live births and the under-five mortality rate (U5MR) will be 78, considerably above the MDG4 targets of 33 and 43 deaths per 1000 births respectively. Pakistan will not achieve the MDG related to nutrition. The review aims to develop a limited set of practical options that will allow the government and other stakeholders to improve the availability and use of health services, especially for the poor. The report mainly focuses on synthesizing the available body of knowledge through review of existing studies, reports and research. The report starts with a description of the health and nutrition status of the population, Pakistan's fertility and growth rates and potential to benefit from a demographic dividend, the burden of diseases and trends in the past few years. The second and third chapters describe the performance of the public health care system over time, including coverage and utilization, quality of care, equity, and patient satisfaction and the impact of national programs. Chapter four discusses the financing of the sector, including overall trends, and the composition of public and private expenditures. Chapter five describes the organization and management of the sector and examines the role of government. Chapter six concludes by summarizing the key challenges facing the sector and suggesting a way forward.Publication Sparing Lives : Better Reproductive Health for Poor Women in South Asia(New Delhi: Macmillan India, Ltd., 2008)In this context, the overall purpose of this review is to bring attention to the opportunities that five countries in the region - Bangladesh, India, Nepal, Pakistan and Sri Lanka - have to strengthen and expand interventions to improve the reproductive health of poor women. The specific objectives are: i) to provide an accurate picture of the current status of women's reproductive health, describe the use of reproductive health services and barriers to use, and identify the improvements required to increase their effectiveness and improve health outcomes; ii) to elucidate individual and household characteristics that affect reproductive health status and use of services so that the most important of these can be used to identify women and households with the greatest need for care to achieve better health; iii) to describe a simple and effective approach - decentralized action planning - that can be used widely in all five countries to improve reproductive health service delivery and outcomes, and point to a body of best practices in reproductive health that provides models and lessons for improvements in South Asia; and iv) to strengthen the case for investing in poor women's reproductive health by demonstrating the links between poverty, inequality, reproductive health care and expenditure.Publication Maternal and Child Survival : Findings from Five Countries Experience in Addressing Maternal and Child Health Challenges(World Bank, Washington, DC, 2014-05)Considerable progress has been made towards the achievement of the Millennium Development Goals (MDGs) since 1990. Although advances in improving MDG 4 and MDG 5a (reducing child and maternal mortality, respectively) have been made, progress is some countries have been insufficient. While some countries have made substantial gains, others have not. This paper is part of a larger study that aims to address this gap in knowledge. The paper discusses the findings from qualitative case studies of five countries that are either on track to meet MDGs 4 and 5a by 2015 or have made significant progress to this end (Bolivia, China, Egypt, Malawi and Nepal). Although they have different socio-economic characteristics, all have made significant advancements due to a strong commitment to improving maternal and child health. To do this, strong political commitment, through policies backed by financial and programmatic support, was critical. In addition, focusing on the most vulnerable populations helped increase access to and use of services. Empowering women and families through education, employment, and poverty reduction programs have led to better health outcomes. These countries still face challenges, however, in terms of the evolving health system, and changes at the economic, social and political levels. Future qualitative and quantitative analyses on the returns of health investments, the political context and institutional arrangements at the country level could help deepen the understanding of the ways in which various countries, with their unique conditions, can improve MCH.Publication Getting to Equal(Washington, DC, 2011)To achieve gender equality and empower women, it is essential to invest in human development. The World Development Report 2012: Gender Equality and Development (hereafter WDR 2012) brings the best global evidence to bear on the relationship between gender equality and development. A central theme running through the report is how investments and outcomes in human development namely health, education, social protection, and labor shape, and are shaped by, gender equality. This note is designed as a companion to the WDR 2012 and highlights some of the World Bank Group's recent experience with and impact on promoting gender equality through its work in human development. Gender equality benefits society as a whole. Greater gender equality today shapes the norms and cultures as well as the constraints and possibilities of tomorrow's men and women. A wealth of evidence demonstrates that gender equality begins a virtuous circle of higher productivity, lower poverty, and better development outcomes for generations to come.

Publication Poverty and Shared Prosperity 2020(Washington, DC: World Bank, 2020-10-07)Previous Poverty and Shared Prosperity Reports have conveyed the difficult message that the world is not on track to meet the global goal of reducing extreme poverty to 3 percent by 2030. This edition brings the unwelcome news that COVID-19, along with conflict and climate change, has not merely slowed global poverty reduction but reversed it for first time in over twenty years. With COVID-19 predicted to push up to 100 million additional people into extreme poverty in 2020, trends in global poverty rates will be set back at least three years over the next decade. Today, 40 percent of the global poor live in fragile or conflict-affected situations, a share that could reach two-thirds by 2030. Multiple effects of climate change could drive an estimated 65 to 129 million people into poverty in the same period. “Reversing the reversal” will require responding effectively to COVID-19, conflict, and climate change while not losing focus on the challenges that most poor people continue to face most of the time. Though these are distinctive types of challenges, there is much to be learned from the initial response to COVID-19 that has broader implications for development policy and practice, just as decades of addressing more familiar development challenges yield insights that can inform responses to today’s unfamiliar but daunting ones. Solving novel problems requires rapid learning, open cooperation, and strategic coordination by everyone: from political leaders and scientists to practitioners and citizens.Publication A Blue Transformation for Pacific Maritime Transport: Overarching Regional Transport(World Bank, Washington, DC, 2023-06-29)This report has eight chapters. Following the introduction (Pacific Peoples and the Sea), the next six chapters each focus on a separate significant component of Pacific maritime transport, analyzing the major influences and challenges, and, where relevant, key areas for future attention. The topics are: international shipping, gateway ports, domestic maritime transport, four related sectors, cruise ship tourism, tuna fisheries, fossil fuel imports, and bulk shipping, natural disasters and climate resilience, and sector governance and institutions. The final chapter, transforming pacific maritime transport, ways forward, distils the report’s findings into the most significant and far-reaching opportunities to transform maritime transport in the Pacific. These are grouped into three broad themes, infrastructure, services, and governance and capacity building. Ways Forward comes at the end and, for readers unable to view the whole report, is a good place to begin. The rest of this executive summary explains why the Pacific is a special case for investment and provides a summary of the main chapters and findings. But first, it describes which Pacific Island countries contributed to the study.Publication Global Economic Prospects, January 2025(Washington, DC: World Bank, 2025-01-16)Global growth is expected to hold steady at 2.7 percent in 2025-26. However, the global economy appears to be settling at a low growth rate that will be insufficient to foster sustained economic development—with the possibility of further headwinds from heightened policy uncertainty and adverse trade policy shifts, geopolitical tensions, persistent inflation, and climate-related natural disasters. Against this backdrop, emerging market and developing economies are set to enter the second quarter of the twenty-first century with per capita incomes on a trajectory that implies substantially slower catch-up toward advanced-economy living standards than they previously experienced. It replaces and improves upon the Doing Business project. B-READY provides a comprehensive data set and description of the factors that strengthen the private sector, not only by advancing the interests of individual firms but also by elevating the interests of workers, consumers, potential new enterprises, and the natural environment. This 2024 report introduces a new analytical framework that benchmarks economies based on three pillars: Regulatory Framework, Public Services, and Operational Efficiency. The analysis centers on 10 topics essential for private sector development that correspond to various stages of the life cycle of a firm. The report also offers insights into three cross-cutting themes that are relevant for modern economies: digital adoption, environmental sustainability, and gender. B-READY draws on a robust data collection process that includes specially tailored expert questionnaires and firm-level surveys. The 2024 report, which covers 50 economies, serves as the first in a series that will expand in geographical coverage and refine its methodology over time, supporting reform advocacy, policy guidance, and further analysis and research.Publication Doing Business 2020(Washington, DC: World Bank, 2020)Doing Business 2020 is the 17th in a series of annual studies investigating the regulations that enhance business activity and those that constrain it.
