Improving the Quality of Frontline Nutrition Services in Indonesia's Health Sector

Abstract
In the past two decades, Indonesia significantly improved its economic growth, poverty, and maternal and child health outcomes. Despite these notable achievements, the country's rates of stunting and malnutrition are among the highest in the world and threaten early childhood development as the stepping-stone of human capital formation. Though government guidelines, standards, and training have helped improve nutrition services in the health sector, there continues to be considerable variation in service quality across districts, between urban and rural areas, and among public and private facilities, with many mothers and children being provided suboptimal services. Malnutrition is a multisectoral issue that is not the “problem” of the health sector alone. However, many of the high-impact health interventions known to improve nutrition outcomes for children are not being implemented in Indonesia, calling for a higher-quality health system to produce better nutrition outcomes. This report analyzes the opportunities to improve the quality of frontline nutrition interventions in Indonesia’s health sector as an element of achieving the National Strategy to Accelerate Stunting Prevention (2018–2021) (StraNas Stunting) goals. It uses a framework adapted from the Lancet Global Health Commission’s report on High-Quality Health Systems in the Sustainable Development Goals Era, which explains that improving the quality of nutrition health care requires system-wide action. In specific, high-quality nutrition services necessitate both process and foundational reforms at the macro, meso, and micro levels. The paper outlines the challenges and proposes recommendations to improve quality nutrition care and services in the country. These are related to strong leadership, harmonized guidelines and targets, and robust regulatory and quality improvement mechanisms; improved monitoring and evaluation and data use; predictable, adequate, and timely financing; platforms for care; competent health workers and a sound supportive supervision system; adequate supplies and functional equipment; and systems that respond to clients’ health needs and expectations.

    A high-quality health systems approach that integrates nutrition is vital to accelerate progress in nutrition and meet the sustainable development goals by 2030.High-quality health systems for nutrition include integrated service provision and supplies, performance monitoring, strategic purchasing, and functioning referral services. Underpinning these components are political leadership and commitment, well-defined quality metrics and quality and timely nutrition data, and an accountability system that nurtures demand for quality services, among others. Several World Bank and Global Financing Facility (GFF) - co-financed projects are investing in building high-quality health system foundations to improve the quality of nutrition services and can serve as examples for improving quality of nutrition care.

    Improving Child Nutrition Outcomes in India : Can the Integrated Child Development Services Program Be More Effective?

    Levels of child malnutrition in India fell only slowly during the 1990s, despite significant economic growth and large public spending on the Integrated Child Development Services (ICDS) program, of which the major component is supplementary feeding for malnourished children. To unravel this puzzle, the authors assess the program's placement and its outcomes using National Family Health Survey data from 1992 and 1998. They find that program placement is clearly regressive across states. The states with the greatest need for the program - the poor northern states with high levels of child malnutrition and nearly half of India's population - have the lowest program coverage and the lowest budgetary allocations from the central government. Program placement within a state is more progressive: poorer and larger villages have a higher probability of having an ICDS center, as do those with other development programs or community associations. The authors also find little evidence of program impact on child nutrition status in villages with ICDS centers.

    Child Nutrition, Economic Growth, and the Provision of Health Care Services in Vietnam in the 1990s

    Vietnam's rapid economic growth in the 1990s greatly increased the incomes of Vietnamese households, which led to a dramatic decline in poverty. Over the same period, child malnutrition rates in Vietnam, as measured by low height for age in children under 5, fell from 50 percent in 1992-93 to 34 percent in 1997-98. Disparities exist, however, between different regions, urban and rural areas, ethnicities, and income quintiles. This dramatic improvement in child nutrition during a time of high economic growth suggests that the nutritional improvements are due to higher household incomes. The authors investigate whether this causal hypothesis is true by estimating the impact of household income growth on children's nutritional status in Vietnam. Different estimation methods applied to the 1992-93 and 1997-98 Vietnam Living Standards Survey data find that growth in household expenditures accounts for only a small proportion of the improvements in children's nutritional status. The authors use data on local health facilities to investigate the role that they may have played in raising children's nutritional status in Vietnam.

    Prospects for Improving Nutrition in Eastern Europe and Central Asia

    Under-nutrition, and micronutrient deficiencies are critical issues for Eastern Europe and Central Asia. Failure to target programs to address the impact of malnutrition, has led to high rates of under-nutrition, and iron and iodine deficiencies. This report highlights the life-threatening nutrition problems confronting people in the region, and suggests strategies for change: a) improving infant feeding, particularly in Central Asia, to reduce unacceptably high rates of under-nutrition in young children; b) increasing consumption of iodized salt, to reduce compromised intelligence levels; and, c) developing programs to reduce iron deficiency anemia in women, and young children. Policymakers, and healthcare providers must develop, and deliver consistent, persuasive messages. This book aims to help raise awareness of nutrition problems in the region, and, suggests solutions for reducing malnutrition.

    Situational Analysis Improving Economic Outcomes by Expanding Nutrition Programming in Tajikistan

    Undernutrition in Tajikistan remains an important public health challenge, albeit a hidden problem. Stunting, iodine deficiency, and maternal and child anemia represent the largest burden of undernutrition in Tajikistan. In 2009, around 29 percent of children in all regions of the country were stunted. Iodine deficiency was observed in 53 percent of children and in 58.6 percent of women. The national prevalence of anemia in children was 28.8 percent; however, rates were as high as 39.8 percent in Ghorno-Badakhshan Autonomous Province and in 32 percent directly ruled districts (DRD). The prevalence of anemia among mothers was 24.2 percent. The long-term effects of these conditions negatively affect the health of adults throughout their life, as well as their potential productivity in the work force and possible economic contribution to the nation. The highest priority interventions will improve infant and young child feeding. Strengthening and scaling up breastfeeding promotion will save lives and help to reduce stunting as would complementary feeding for babies six months and older. Promoting exclusive breastfeeding for infants under six months is the most efficacious intervention to save lives, averting nearly 20 percent of deaths in children under-five. Effective programs need to be designed to make more women aware of the benefits of breastfeeding and sound nutrition. A comprehensive, multi-sector approach is needed to ensure success of these interventions. This report also makes several other recommendations including: supplementing pregnant women with either iron folic acid or multiple micronutrients, maintain twice-annually vitamin A supplementation, implementing a deworming program, support for flour fortification, scale up and maintain zinc for the management of diarrhea, and address the underlying and basic causes of undernutrition through other sectors.

    The global economy is facing another substantial headwind, emanating largely from an increase in trade tensions and heightened global policy uncertainty. For emerging market and developing economies (EMDEs), the ability to boost job creation and reduce extreme poverty has declined. Key downside risks include a further escalation of trade barriers and continued policy uncertainty. These challenges are exacerbated by subdued foreign direct investment into EMDEs. Global cooperation is needed to restore a more stable international trade environment and scale up support for vulnerable countries grappling with conflict, debt burdens, and climate change. Domestic policy action is also critical to contain inflation risks and strengthen fiscal resilience. To accelerate job creation and long-term growth, structural reforms must focus on raising institutional quality, attracting private investment, and strengthening human capital and labor markets. Countries in fragile and conflict situations face daunting development challenges that will require tailored domestic policy reforms and well-coordinated multilateral support.

    Interest Rate Pass-Through

    The interest rate pass-through describes how changes in a reference rate (the monetary policy, money market, or T-bill rate) transmit to bank lending rates. This paper reviews the empirical literature on the interest rate pass-through and systematizes it by means of meta-analysis and meta-regressions. The paper finds systematically lower estimated pass-through coefficients in studies that focus on transmission to long-term lending rates, consumer lending rates, and average lending rates. The interest rate pass-through is significantly influenced by country macro-financial and institutional factors. The estimated pass-through tends to be stronger for economies with deeper capital markets (measured by market capitalization). Interestingly, central bank independence rising from lower levels can reduce interest rate pass-through, while central bank independence rising from already high levels can boost the pass-through.

Global growth is expected to slow further this year, reflecting the lagged and ongoing effects of tight monetary policy to rein in inflation, restrictive credit conditions, and anemic global trade and investment. Downside risks include an escalation of the recent conflict in the Middle East, financial stress, persistent inflation, weaker-than-expected activity in China, trade fragmentation, and climate-related disasters. Against this backdrop, policy makers face enormous challenges. In emerging market and developing economies (EMDEs), commodity exporters face the enduring challenges posed by fiscal policy procyclicality and volatility, which highlight the need for robust fiscal frameworks. Across EMDEs, previous episodes of investment growth acceleration underscore the critical importance of macroeconomic and structural policies and an enabling institutional environment in bolstering investment and long-term growth. At the global level, cooperation needs to be strengthened to provide debt relief, facilitate trade integration, tackle climate change, and alleviate food insecurity.

    All African countries need better and more jobs for their growing populations. "Digital Africa: Technological Transformation for Jobs" shows that broader use of productivity-enhancing, digital technologies by enterprises and households is imperative to generate such jobs, including for lower-skilled people. At the same time, it can support not only countries’ short-term objective of postpandemic economic recovery but also their vision of economic transformation with more inclusive growth. These outcomes are not automatic, however. Mobile internet availability has increased throughout the continent in recent years, but Africa’s uptake gap is the highest in the world. Areas with at least 3G mobile internet service now cover 84 percent of Africa’s population, but only 22 percent uses such services. And the average African business lags in the use of smartphones and computers as well as more sophisticated digital technologies that catalyze further productivity gains. Two issues explain the usage gap: affordability of these new technologies and willingness to use them. For the 40 percent of Africans below the extreme poverty line, mobile data plans alone would cost one-third of their incomes—in addition to the price of access devices, apps, and electricity. Data plans for small- and medium-size businesses are also more expensive than in other regions. Moreover, shortcomings in the quality of internet services—and in the supply of attractive, skills-appropriate apps that promote entrepreneurship and raise earnings—dampen people’s willingness to use them. For those countries already using these technologies, the development payoffs are significant. New empirical studies for this report add to the rapidly growing evidence that mobile internet availability directly raises enterprise productivity, increases jobs, and reduces poverty throughout Africa. To realize these and other benefits more widely, Africa’s countries must implement complementary and mutually reinforcing policies to strengthen both consumers’ ability to pay and willingness to use digital technologies. These interventions must prioritize productive use to generate large numbers of inclusive jobs in a region poised to benefit from a massive, youthful workforce—one projected to become the world’s largest by the end of this century.
