Abstract
The largest global equity gap in health, is among children, and concentrated in communicable diseases. This note examines the work of the Integrated Management of Childhood Illness (IMCI) - a strategy to improve child health outcomes, developed by the World Health Organization, and the United Nations Children's Fund - whose aim is to reduce childhood deaths, illnesses, and disabilities, while improving children's growth, and development. IMCI's three main components are to: improve family, and community practices related to child health, and nutrition; improve the health system for effective management of childhood illnesses; and, improve health workers' skills. Finally, the note highlights further needs, e.g., creating working groups to develop national plans in accordance with the three IMCI components, adapting national guidelines, with the IMCI training material, and include the major causes of child deaths, and conditions; targeting poor communities, and disadvantaged children; and, linking health facilities with communities, while ensuring appropriate, and affordable referral arrangements.

    Clearing the Global Health Fog : A Systematic Review of the Evidence on Integration of Health Systems and Targeted Interventions

    A longstanding debate on health systems organization relates to benefits of integrating health programs that emphasize specific interventions into mainstream health systems to increase access and improve health outcomes This paper is organized in five chapters. This introduction is followed by the methodology chapter, which includes a brief section on the conceptual framework used to analyze the studies retrieved and the programs presented within these to map the nature and extent of integration into critical health system functions. The results chapter includes: a summary of the outcomes for each study grouped by the disease area or the clinical problem the intervention seeks to address, including the reported success; for each program, analysis and mapping of the nature and extent of integration into critical health system functions; and an analysis of how contextual factors either created opportunities for introducing or integrating a program or influenced the desirability or feasibility of program integration. The discussion chapter provides an overview of the implication of findings for policy makers, practitioners and researchers. The final chapter draws conclusions.

    Public Health Surveillance Toolkit : A Guide for Busy Task Managers

    This toolkit draws on the expertise of public health practitioners who have experience with public health surveillance and who have recognized the core role of surveillance in public health. These practitioners have advocated for surveillance programs, supplied innovative ideas, and provided insightful critiques over many years. This toolkit also draws on the experience of Bank staff and technical experts from the PAHO and the CDC who have contributed to Bank missions. The toolkit also makes use of WHO references, primarily those from the WHO s Web site. Part A of this toolkit provides some theoretical concepts, and knowledge about surveillance that has been gained through applying these concepts and the practice of surveillance in developing countries. Part B provides information that will be useful to Task Managers as they prepare loans for strengthening public health surveillance systems. Several World Bank experiences are shared. The focus of part B is on practical aspects of surveillance and on lessons learned.

    Angola Nutrition Gap Analysis

    Despite remarkable economic growth in the past decade, undernutrition remains a serious public health problem in Angola. High rates of child stunting and micronutrient deficiencies are contributing to an under-five mortality rate of 161 deaths per 1,000 live births, limiting the growth and development of children, hindering productivity, and preventing the country from reaching millennium development goals one and four. In response to this situation, the objectives of this report are to: a) synthesize available information on the nutrition situation in Angola; b) discuss the three main determinants of undernutrition using the United Nations Children's Fund (UNICEF) conceptual framework; c) summarize existing nutrition policies and programs in the country; and d) propose next steps for action. Nutrition policy agenda is slowly gaining momentum in the country. The national food security and nutrition strategy released in 2009 include nutrition actions for children under five. The national nutrition policy is in the process of being finalized. High priority problems include the dearth of up-to-date, reliable, and comprehensive information on the nutrition situation in the country, severe shortages of trained nutritionists, and an exclusion of nutrition from community-based health activities. The following recommendations are proposed as next steps: 1) incorporate the collection of nutrition data into routine, planned surveys; 2) finalize the national nutrition policy, conduct a rigorous gap analysis, and develop a costed implementation plan to effectively scale-up nutrition activities in the country; 3) enhance capacity building efforts in the field of nutrition; 4) improve nutrition surveillance and screening activities and; 5) incorporate nutrition into community-based activities.

    Nutrition Policy and Programs in Ghana

    Although Sub-Saharan Africa has some of the worst nutrition indicators in the world, nutrition remains a low priority on the policy agendas of many African governments. This despite the fact that proven interventions are known and available and that investment in them is considered a cost-effective strategy for poverty reduction. This case study is one in a series seeking to understand (1) what keeps African governments from committing fully to reducing malnutrition, and (2) what is required for full commitment. It documents how the Ghanaian government has addressed the issue of malnutrition since Independence, examines what political and institutional factors have prevented full commitment, and identifies what conditions have moved the nutrition agenda forward at different points in time. The primary objective of this study as well as the series as a whole is to help African governments, development partners, and nutrition and health practitioners identify, understand and address the political and institutional obstacles preventing sustainable progress in nutrition.

    Blood Services in Central Asian Health Systems : A Clear and Present Danger of Spreading HIV/AIDS and Other Infectious Diseases

    The report discusses inter-related parts of blood transfusions systems, and presents an overview of the parts that need to be strengthened in Central Asia. Numerous parts are in serious need of organizational restructuring, new investment and increased budgetary support for operation and maintenance. This report sets them out such that each can be addressed in turn and some simultaneously. The report also discusses the health threat posed by alarmingly low levels of blood supplies, fostered by a culture that places little value on donating blood, public fear of being infected by giving blood, and the near absence of donor promotion campaigns.

    South Asia Development Update, October 2024: Women, Jobs, and Growth

    South Asia’s growth is on track to exceed earlier expectations, in a broad-based upturn. The region is expected to remain the fastest-growing among emerging market and developing economies (EMDEs). Several risks could upend this generally promising outlook, including extreme weather events, social unrest, and policy missteps, such as reform delays. But South Asian countries also have considerable untapped potential that could help them further boost productivity growth and employment and adapt to climate change. In particular, with about two-thirds of the region’s working-age women out of the labor force, raising female employment rates to those of men could increase per capita income by as much as one-half. Measures to accelerate job creation, remove obstacles to women working, and equalize gender rights would be more effective if combined with a shift toward social norms that looked more favorably on working women. Also, most South Asian countries rank among the EMDEs least open to global trade and investment. Greater openness could boost women’s employment, spur the growth of firms, and allow the region to take better advantage of the reshaping of global supply chains and trade. Reducing the cost of conducting business could help the region better harness large-scale remittance inflows.

    All African countries need better and more jobs for their growing populations. "Digital Africa: Technological Transformation for Jobs" shows that broader use of productivity-enhancing, digital technologies by enterprises and households is imperative to generate such jobs, including for lower-skilled people. At the same time, it can support not only countries’ short-term objective of postpandemic economic recovery but also their vision of economic transformation with more inclusive growth. These outcomes are not automatic, however. Mobile internet availability has increased throughout the continent in recent years, but Africa’s uptake gap is the highest in the world. Areas with at least 3G mobile internet service now cover 84 percent of Africa’s population, but only 22 percent uses such services. And the average African business lags in the use of smartphones and computers as well as more sophisticated digital technologies that catalyze further productivity gains. Two issues explain the usage gap: affordability of these new technologies and willingness to use them. For the 40 percent of Africans below the extreme poverty line, mobile data plans alone would cost one-third of their incomes—in addition to the price of access devices, apps, and electricity. Data plans for small- and medium-size businesses are also more expensive than in other regions. Moreover, shortcomings in the quality of internet services—and in the supply of attractive, skills-appropriate apps that promote entrepreneurship and raise earnings—dampen people’s willingness to use them. For those countries already using these technologies, the development payoffs are significant. New empirical studies for this report add to the rapidly growing evidence that mobile internet availability directly raises enterprise productivity, increases jobs, and reduces poverty throughout Africa. To realize these and other benefits more widely, Africa’s countries must implement complementary and mutually reinforcing policies to strengthen both consumers’ ability to pay and willingness to use digital technologies. These interventions must prioritize productive use to generate large numbers of inclusive jobs in a region poised to benefit from a massive, youthful workforce—one projected to become the world’s largest by the end of this century.

    The Country Opinion Survey in China assists the World Bank Group (WBG) in better understanding how stakeholders in China perceive the WBG. It provides the WBG with systematic feedback from national and local governments, multilateral/bilateral agencies, media, academia, the private sector, and civil society in China on 1) their views regarding the general environment in China; 2) their overall attitudes toward the WBG in China; 3) overall impressions of the WBG’s effectiveness and results, knowledge work and activities, and communication and information sharing in China; and 4) their perceptions of the WBG’s future role in China.

    The World Bank Group in Georgia, 2014-23

    This Country Program Evaluation assesses the performance and effectiveness of the World Bank Group’s support to Georgia in achieving the country’s development objectives. In the decade leading up to the evaluation period, Georgia pursued economic reforms to attract critical investments for becoming a regional trade and transport hub. Ambitious economic reforms went hand in hand with efforts to improve human development and strengthening social protection systems. Growing geopolitical tensions and internal political polarization have challenged Georgia’s reform progress in recent years. The Bank Group’s strategy adapted well to Georgia’s development needs and was well coordinated with other development partners. It successfully employed a range of instruments to help increase competitiveness, growth, and job creation, and effectively contributed to improved infrastructure and increased trade by using programmatic and innovative approaches. The Bank Group’s regular investments in analytical work and the switch to results-based programmatic support helped improve the efficiency and effectiveness of education and health care systems. The IEG offers the following lessons based on the evidence and analysis in the Country Program Evaluation: (i) Prioritizing Bank Group support around the move towards deeper regional integration was an effective anchor for key economic reforms for economic convergence. (ii) Pursuing a selective and adaptive approach in a country with high implementation capacity and institutions, strong coordination among development partners, and access to a wide range of external resources can allow the Bank Group to exercise significant influence in areas of comparative advantage and global expertise. (iii) A stronger focus on outcome-based programmatic approaches helped to build local capacity and crowd-in partner financing.

    An Investment Framework for Nutrition

    The report estimates the costs, impacts and financing scenarios to achieve the World Health Assembly global nutrition targets for stunting, anemia in women, exclusive breastfeeding and the scaling up of the treatment of severe wasting among young children. To reach these four targets, the world needs $70 billion over 10 years to invest in high-impact nutrition-specific interventions. This investment would have enormous benefits: 65 million cases of stunting and 265 million cases of anemia in women would be prevented in 2025 as compared with the 2015 baseline. In addition, at least 91 million more children would be treated for severe wasting and 105 million additional babies would be exclusively breastfed during the first six months of life over 10 years. Altogether, achieving these targets would avert at least 3.7 million child deaths. Every dollar invested in this package of interventions would yield between $4 and $35 in economic returns, making investing in early nutrition one of the best value-for-money development actions. Although some of the targets—especially those for reducing stunting in children and anemia in women—are ambitious and will require concerted efforts in financing, scale-up, and sustained commitment, recent experience from several countries suggests that meeting these targets is feasible. These investments in the critical 1000 day window of early childhood are inalienable and portable and will pay lifelong dividends – not only for children directly affected but also for us all in the form of more robust societies – that will drive future economies.
