Abstract
This report assesses Indonesia’s health financing system. As an intrinsic and necessary element of universal health coverage (UHC), health financing is not only about assessing the sufficiency of resources, but also about how equitably and efficiently resources are raised, pooled, and allocated to make progress towards UHC.

    This report identifies critical opportunities and constraints faced by Solomon Islands health system as it makes progress towards Universal Health Coverage. Protecting the achievements that have been made over recent years, while continuing to improve a broader range of health outcomes, requires effective management of changes in financial and other institutional arrangements in the health sector. Solomon Islands has made some impressive gains in health outcomes over the last two decades. Continued improvements will require more effective governance to manage finite resources at all health service levels. Now more than ever, with decreasing real per capita health expenditure, greater focus is needed on spending health dollars better. More efficiency. More quality of service delivery. More targeted interventions for vulnerable groups to improve equity.

    Vanuatu Health Financing Systems Assessment

    This report identifies critical opportunities and constraints faced by the Vanuatu health system as it makes progress toward universal health coverage (UHC). Protecting the achievements that have been made over recent years, while continuing to improve a broader range of health outcomes, requires effective management of changes in financial and other institutional arrangements in the health sector. This health financing systems assessment (HFSA) provides context and emphasizes key issues and messages aimed at providing Ministry of Health (MoH) and partners with a platform to help with this transition. It builds upon other work that has been done in recent years on health in Vanuatu. The assessment begins with an overview of the prevailing economic and fiscal context and key health outcomes. It then overviews the country’s health system - through its human, physical, and financial dimensions. Finally, it reviews health financing in some detail to identify trends across time; against regional benchmarks; and at both national and subnational and facility levels. The assessment concludes with a summary of policy options.

    Health Financing in Indonesia : A Reform Road Map

    Indonesia is at a critical stage in the development and modernization of its health system. The government of Indonesia has made major improvements over the past four decades, but struggles to maintain and continue to improve important health outcomes for the poor and achieve the Millennium Development Goals. Nevertheless, some key health indicators show significant progress. Infant and child (under five) mortality rates have fallen by half since the early 1990s, although the speed of the decline appears to have slowed since 2002. Maternal mortality rates show a declining trend, but remain among the highest in East Asia. Indonesia's population program is one of the worlds most successful: fertility rates have declined impressively since the 1970s and continue to fall. Previously declining malnutrition rates among young children have, however, stagnated. The slowing down of progress may be explained by a poorly functioning health system as well as by new and ongoing challenges posed by demographic, epidemiological, and nutrition transitions, which require new policy directions, a reconfigured and better performing health system, and long-term sustainable financing.

    The Nuts & Bolts of Jamkesmas, Indonesia’s Government-Financed Health Coverage Program for the Poor and Near-Poor

    This case study describes and assesses Jamkesmas, Indonesia's government-financed health coverage program for the poor and near-poor. It provides a detailed description of the scope, depth, and breadth of coverage provided under Jamkesmas, and highlights ways in which the program interacts with the rest of Indonesia's health system. It also summarizes and discusses evidence on whether Jamkesmas is attaining its stated objectives of removing financial barriers and improving access to health care by the poor and near-poor, what could be improved, and what lessons can be learned from the experience of Jamkesmas that could help inform Indonesia's quest for universal coverage. The primary theme underlying the study is that supply-side constraints and supply-side subsidies have not been leveraged to increase the effectiveness of the Jamkesmas program. There are significant geographic deficiencies in the availability and quality of the basic benefits package, especially for those living in relatively remote and rural locations of the country, and this limits the effective availability of benefits for many Jamkesmas beneficiaries. The remainder of the case study is organized as follows. Section two provides general background and information on health system outcomes in Indonesia. Section three is an overview of health care financing and delivery. Section four describes the institutional architecture of Jamkesmas. Section five highlights the process of targeting, identification, and enrolment of beneficiaries under the program. Section six focuses on the role of public financing. Section seven outlines the basic benefits package. Section eight provides an overview of the information environment of Jamkesmas. Section nine discusses the special theme of supply-side constraints and supply-side subsidies that dilute the effectiveness of the Jamkesmas program. Section ten discusses the pending agenda around some of the architectural and operational features of Jamkesmas in the context of universal coverage.

    Kiribati Health Financing System Assessment

    This report identifies critical opportunities and constraints faced by the Kiribati health system as it makes progress towards Universal Health Coverage. Protecting the achievements that have been made over recent years, while continuing to improve a broader range of health outcomes, requires effective management of changes in financial and other institutional arrangements in the health sector. Despite notable improvements in recent decades, most population health outcomes in Kiribati do not compare well with other small countries in the Pacific. In many respects, this reflects the very difficult geographic, environmental and social determinants of health that the country faces. Strong collaboration across sectors is essential to implement the commitment to “health in all Government policies” agreed to in the Ambo Declaration in 2017. The Ministry of Health and Medical Services has taken impressive steps to improve its oversight of health sector performance. Continued attention to spending health dollars better is needed to achieve more efficient and quality outcomes from frontline service delivery, including for the outer islands.

    Government Matters III : Governance Indicators for 1996-2002

    The authors present estimates of six dimensions of governance covering 199 countries and territories for four time periods: 1996, 1998, 2000, and 2002. These indicators are based on several hundred individual variables measuring perceptions of governance, drawn from 25 separate data sources constructed by 18 different organizations. The authors assign these individual measures of governance to categories capturing key dimensions of governance and use an unobserved components model to construct six aggregate governance indicators in each of the four periods. They present the point estimates of the dimensions of governance as well as the margins of errors for each country for the four periods. The governance indicators reported here are an update and expansion of previous research work on indicators initiated in 1998 (Kaufmann, Kraay, and Zoido-Lobat 1999a,b and 2002). The authors also address various methodological issues, including the interpretation and use of the data given the estimated margins of errors.

    Governance Matters VIII : Aggregate and Individual Governance Indicators 1996–2008

    This paper reports on the 2009 update of the Worldwide Governance Indicators (WGI) research project, covering 212 countries and territories and measuring six dimensions of governance between 1996 and 2008: Voice and Accountability, Political Stability and Absence of Violence/Terrorism, Government Effectiveness, Regulatory Quality, Rule of Law, and Control of Corruption. These aggregate indicators are based on hundreds of specific and disaggregated individual variables measuring various dimensions of governance, taken from 35 data sources provided by 33 different organizations. The data reflect the views on governance of public sector, private sector and NGO experts, as well as thousands of citizen and firm survey respondents worldwide. The authors also explicitly report the margins of error accompanying each country estimate. These reflect the inherent difficulties in measuring governance using any kind of data. They find that even after taking margins of error into account, the WGI permit meaningful cross-country comparisons as well as monitoring progress over time. The aggregate indicators, together with the disaggregated underlying indicators, are available at www.govindicators.org.

    Governance Matters IV : Governance Indicators for 1996-2004

    The authors present the latest update of their aggregate governance indicators, together with new analysis of several issues related to the use of these measures. The governance indicators measure the following six dimensions of governance: (1) voice and accountability; (2) political instability and violence; (3) government effectiveness; (4) regulatory quality; (5) rule of law, and (6) control of corruption. They cover 209 countries and territories for 1996, 1998, 2000, 2002, and 2004. They are based on several hundred individual variables measuring perceptions of governance, drawn from 37 separate data sources constructed by 31 organizations. The authors present estimates of the six dimensions of governance for each period, as well as margins of error capturing the range of likely values for each country. These margins of error are not unique to perceptions-based measures of governance, but are an important feature of all efforts to measure governance, including objective indicators. In fact, the authors give examples of how individual objective measures provide an incomplete picture of even the quite particular dimensions of governance that they are intended to measure. The authors also analyze in detail changes over time in their estimates of governance; provide a framework for assessing the statistical significance of changes in governance; and suggest a simple rule of thumb for identifying statistically significant changes in country governance over time. The ability to identify significant changes in governance over time is much higher for aggregate indicators than for any individual indicator. While the authors find that the quality of governance in a number of countries has changed significantly (in both directions), they also provide evidence suggesting that there are no trends, for better or worse, in global averages of governance. Finally, they interpret the strong observed correlation between income and governance, and argue against recent efforts to apply a discount to governance performance in low-income countries.

    Breaking the Conflict Trap : Civil War and Development Policy

    Most wars are now civil wars. Even though international wars attract enormous global attention, they have become infrequent and brief. Civil wars usually attract less attention, but they have become increasingly common and typically go on for years. This report argues that civil war is now an important issue for development. War retards development, but conversely, development retards war. This double causation gives rise to virtuous and vicious circles. Where development succeeds, countries become progressively safer from violent conflict, making subsequent development easier. Where development fails, countries are at high risk of becoming caught in a conflict trap in which war wrecks the economy and increases the risk of further war. The global incidence of civil war is high because the international community has done little to avert it. Inertia is rooted in two beliefs: that we can safely 'let them fight it out among themselves' and that 'nothing can be done' because civil war is driven by ancestral ethnic and religious hatreds. The purpose of this report is to challenge these beliefs.
